Akut meningokoksemili olguda gitelman sendromu
Meningokokkal hastalığın geniş bir klinik spektrumu vardır; klinik tablo, ateş ve gizli bakteriyemiden sepsis, şok ve ölüme kadar farklılık gösterebilir. Akut menin-gokoksemi, Neisseria meningitis'e bağlı sık rastlanan klinik tablolardan biridir. Özgül olmayan üst solunum yolu infeksiyonu bulguları, gastrointestinal sistem bulguları ve genellikle purpura veya makulopapüler döküntü ile karşımıza çıkabilir. Gitelman sendromu, hipokalemik metabolik alkaloz ve hipomagnezemi ile karakterize otozomal resesif geçişli bir hastalıktır. Bu yazıda, ateş yükseldiği, miyalji, peteşiyel döküntülerle başvuran ve muayene sırasında karpo-pedal spazm saptanan 7,5 yaşında kız olguda, hafif seyirli akut meningokoksemi ve Gitelman sendromu tanısı konulmuş ve tartışılmak istenmiştir.
Gitelman's syndrome in a patient with acute meningococcemia
The spectrum of meningococcal disease can vary widely, from fever and occult bacteremia to sepsis, shock, and death. Acute meningococcemia is a frequent condition. It might present initially as upper respiratory or gastrointestinal infection; maculopapular rash, petechiae and/or purpura can be evident. Gitelman syndrome is an autosomal recessive disorder characterized by hypokalemic metabolic alkalosis and hypomagnesemia. In this paper, we discussed a 7.5 year-old female patient who presented with carpo-pedal spasm, myalgia, petechial rash and fever and diagnosed as acute meningococcemia and Gitelman syndrome.
