Çocukluk çağı santral sinir sitemi enfeksiyonlarında viral etiyolojik ajanların değerlendirilmesi
Amaç: Viral santral sinir sistemi enfeksiyonları çok sayıda ajan tarafından oluşturulabilmektedir. Bu enfeksiyonların etiyolojik tanısı önemlidir, çünkü giderek artan sayıda viral santral sinir sistemi enfeksiyonu antiviral ilaçlarla tedavi edilebilmektedir. Bu çalışmada viral santral sinir sistemi enfeksiyonu düşünülen çocuklarda beyin-omurilik sıvısında "enzyme-immune-assay" yöntemi ile viral patojenlere karşı spesifik antikor araştırılmıştır. Gereç ve yöntem: Çalışmaya öykü, fizik inceleme ve beyin-omurilik sıvısı bulguları ile viral santral sinir sistemi enfeksiyonu tanısı konmuş, yaşları 1-15 arasında değişen (ortalama 7,5±4,8), 13'ü erkek, 9'u kız 22 olgu alınmıştır. Beyin-omurilik sıvısındaki serolojik incelemeler hastalığın belirtilerinin başlamasını takiben ilk bir hafta içinde yapılmıştır. Bulgular: Beyin-omurilik sıvılarında en sık olarak adenoviruslara spesifik antikor saptanmıştır (%38,8). Tesbit edilen diğer antikorlar enteroviruslar (%16,6), para-influenza l (%16,6), herpes simplex virus 1 (%16,6), influenza B (%5,5) ve varicella-zoster virusa (%5,5) spesifik antikorlar olmuştur. Olguların %16'sında etiyolojik herhangi bir ajana özgü antikor tesbit edilmemiştir. Enteroviruslar ve respiratuvar viruslarda tipik mevsimsel dağılım olduğu dikkat çekmiştir. Sonuç: Bu çalışmada kullanılan serolojik yöntem ile çocuklardaki viral santral sinir sistemi enfeksiyonlarının etkene yönelik tanısı ile ilgili bilgi edinilebileceği ve daha fazla sayıda olgunun çalışılması ile daha ayrıntılı epidemiyolojik verilerin elde edilebileceği düşünülmüştür.
The evaluation of the viral etiologic agents in central nervous system infections in childhood
Objective: Viral central nervous system infections may be caused by a number of different agents. It is important to confirm the etiological diagnosis, because an increasing number of central nervous system infections lend themselves to treatment with antiviral drugs. We investigated the specific antibodies against viral pathogens in cerebrospinal fluid in children with suspected viral central nervous system infection using enzyme immunoassay test. Material and method: Twenty-two patients (13 males, 9 females) diagnosed with viral central nervous system infection based on the history, physical examination and laboratory findings were included in the study. Their ages ranged from 1 to 15 years (mean 7.5+4.8). The sérologie examinations were performed in the first week of the illness. Results: The most frequent antibodies detected in cerebrospinal fluid wet adenoviruses (%38.8). Other antibodies detected were for enteroviruses (16.6% parainfluenza 1 (16.6%), herpes simplex virus 1 (16.6%), influenza B virus (5.5% and varicella-zoster virus (5.5%). The causal agent could not be identified in 16% c cases. Characteristic seasonal variation occured associated with entero an respiratory viruses. Conclusions: We concluded that the sérologie method which was applied in ot study could be useful in the diagnosis of the viral agents of the central nervoi system infections and that further studies including more patients could provid more information about the regional epidemiological features of the viral central nervous system infections in childhood.
