Genişletilmiş spektrumlu beta-laktamaz üreten Klebsiella pneumoniae suşlarında imipenem, meropenem ve sefoperazon-sulbaktamin etkinliği
Amaç: Hastane kökenli Klebsiella pneumomiae izolatları arasında, genişlemiş spektrumlu sefalosporinlere karşı giderek artan oranda direnç gözlenmektedir. Bu çalışmanın amacı genişlemiş spektrumlu beta-laktamaz (extended-spectrum beta-lactamase; ESBL) üreten K. pneumoniae suşlarında karbapenemler ve sefoperazon-sulbaktam etkinliğinin araştırılmasıdır. Gereç ve Yöntem: Çalışmaya alınan 159 susun, 62'sinde (%39) çift disk sinerji testi ile ESBL üretimi saptanmıştır. Daha sonra bu suşlar üzerinde İmipenem, meropenem ve sefoperazon-sulbaktamın minimal inhibitör konsantrasyon (MİK) değeri NCCLS önerilerine uyularak mikrodilusyon yöntemi ile belirlendi. Bulgular: İmipenem ve meropenemin, ESBL üreten suşlara karşı etkin antibak-teriyel ajanlar olduğu (dkenç % 0), sefoperazon-sulbaktamın da %16 direnç oranı ile etkin bir antibakteriyel olduğu görüldü. Sonuç: Sonuç olarak genişlemiş spektrumlu sefalosporinlere ve. beta-laktamlara dirençli K. pneumoniae infeksiyonlarında karbapenemler ile sağaltımın yararlı olacağı düşünülmektedir.
Invitro activities of meropenem, imipenem and cefoperazone-sulbactam against extended-spectrum beta-lactamase-producing Klebsiella pneumoniae strains
Objective: Resistance to extended-spectrum cephalosporins is observed at increased frequency among nosocomial isolates of Klebsiella pneumoniae. The aim of the present study was to evaluate the activity of carbapenems and cefoperazone-sulbactam against strains which were confirmed to produce extended-spectrum beta-lactamase(ESBL). Material and method: ESBL-producing strains were determined with double dişe synergy test. ESBL production was detected in 62 of 159 strains (39%). The minimal inhibitory concentrations (MIC) of these strains for İmipenem (IPM), meropenem (MPM) and cefoperazone-sulbactam were determined by using microdilution method according to NCCLS recommendations. Results: Meropenem and İmipenem were the most effective drugs aganist ESBL-producing strains, (resistance rate 0%) and also cefoperazone-sulbactam was also detected to be an effective drug (resistance rate 16%). Conclusion: We conclude that carbapenems are highly effective on Klebsiella pneumoniae strains that are resistant to extended-spectrum cephalosporins and beta-lactams and they may be useful for the treatment of infecüons of these microorganisms.
