Çocukluk çağında nörofibromatozis tip 1 ile ilişkili optik gliom ve yumuşak doku sarkomları
Bu çalışmada, Nörofibromatozis Tip J tanısı ile izlenen hastalar değerlendirilmiş ve iki olguda gelişen malign yumuşak doku sarkomu ile altı olguda saptanan optik glioma olguları sunulmuştur. Nörofibromatozis Tip 1 geninin bir tümör süpressör gen olduğu ve bu gendeki mutasyonların maliyn hastalık gelişimine neden olabileceğini gösteren çalışmalar mevcuttur. Bu hastalarda çocukluk çağı kanseri gelişme riski % 0.27-1 arasında değişmektedir. Toplam 27 hastanın izlendiği bu seride bir rabdomyosarkom ve bir fibrosarkom olgusu izlenmiştir. Bu iki olgu, uygulanan multimodal antikanser tedaviye rağmen, sırasıyla izlemlerinin 58 ve 12. aylarında kaybedilmiştir. Altı olguda (%22.2) optik gliom gelişmiş ve semptomatik tek olguda radyoterapi uygulanmış, asemptomatik olan diğer olgular konservatif olarak izlenmiştir. Nörofibromatozis 1'li bireyler, diğer komplikasyonların yanısıra, malign tümör gelişim riski açısından multidisipliner yaklaşımla izlenmelidir.
In this study, we evaluated children with neurofibromatosis type 1 and presented two cases who developed malignant soft tissue sarcomas and six patients with optic glioma. Neurofibromatosis-1 gene is a tumor suppressor gene and mutations within this gene may lead to development of malignant tumors. The risk of childhood cancer in young patients with neurofibromatosis 1 is 0.27 to 1%. In this study, a total of 27 children with neurofibromatosis type-1 was evaluated and one patient presented with rhabdomyosarcoma and another one developed fibrosarcoma. These two patients received multimodal anticancer therapy but died with malignant disease at 58<sup>th</sup> and 12<sup>th</sup> months of follow-up respectively. Six out of 27 patients (22.2%) had optic glioma. All optic gliomas have been followed up conservatively except for one symptomatic case who received radiotherapy. All patients with neurofibromatosis type-1 should be followed by a multidisciplinary team for the potential complications, especially for the risk of developing malignant tumors.
