Sütçocuğunda iyileşmeyen perianal ülser: Perianal hemanjiyom
Hemanjiyomlar sütçocukluğu döneminin en sık selim tümörüdür. Vakaların % 95'inde tanı hikaye ve fizik muayene ile kolayca konabilir. Ülserasyon en sık görülen komplikasyondur, ancak perianal bölgede ülsere hemanjiyomun görülmesi oldukça nadirdir. Ülserasyon ağrılı olabilir; infeksiyon, kanama ve skar oluşumuna yol açabilir. Ülserasyon için çeşitli tedaviler kullanılmış olmasına rağmen görüş birliği sağlanmış etkin bir tedavi şekli yoktur. Seçilecek tedavi hastanın yaşı, yerleşim, boyut, hemanjiyomun büyüme veya involüsyon fazına bağlıdır. Başlangıç bulgusu perinede Ülserasyon olan ve haftalar sonra hemanjiyom tanısı alan bir vaka sunulmaktadır. Bu vaka perianal hemanjiyomun hayatın ilk günlerinde ülser oluşumu ile karşımıza çıkabileceğine dikkat çekmektedir.
Nonhealing perianal ulcer in a newborn infant: Perianal hemangioma
Hemangiomas are the most common benign tumors of infancy. In 95 % of the cases diagnosis can be established based on history and physical examination alone, Ulceration is the most common local complication of hemangiomas, but the ulcer as the initial presentation of a hemangioma in the perianal region is rare. Ulceration can be painful and can cause infection, hemorrhage, and scarring. . No single, uniformly effective treatment modality exists. The decision to use specific therapies depends on the age of the patient, as well as the location, size, and stage of growth or involution of the hemangioma. We report on infant in whom the initial presentation was a skin ulceration of the perineum with the diagnosis of hemangioma becoming evident to weeks later. This case reminds that perianal hemangioma may present as an ulcer in the first few days or weeks of life.
