SOSYAL ANKSİYETE BOZUKLUĞU HASTALARINDA BEDEN ALGISI VE CİNSEL İŞLEV BOZUKLUKLARININ DEĞERLENDİRİLMESİ

Amaç: Genel olarak anksiyete bozukluklarının cinsel işlevleri olumsuz yönde etkilediği bilinmektedir ancak bu konuda sınırlı sayıda çalışma bulunmaktadır. Ayrıca literatürde Sosyal Anksiyete Bozukluğu (SAB) hastalarında beden algısı ve cinsel işlevleri birlikte değerlendiren bir çalışmaya rastlanmamıştır. Bu çalışmada SAB bulunan kadın hastaların beden algısı ve cinsel işlev bozukluğu (CİB) açısından sağlıklı kontrollerle karşılaştırılması amaçlanmıştır.Gereç ve Yöntemler: Çalışmaya, Bozok Üniversitesi Tıp Fakültesi Hastanesi Ruh Sağlığı ve Hastalıkları Anabilim Dalı ayaktan tedavi ünitesine başvuran, DSM-IV TR tanı ölçütlerine göre SAB tanısı konmuş, düzenli bir cinsel hayatı olan, 18-50 yaş arasındaki 30 kadın hasta ve 28 sağlıklı kadın kontrol dahil edilmiştir. Sosyal anksiyete belirtilerinin şiddetini belirlemek için Liebowitz Sosyal Anksiyete Ölçeği (LSAÖ), cinsel işlevlerin değerlendirilmesinde Arizona Cinsel Yaşantılar Ölçeği (ACYÖ) ve beden algısının değerlendirilmesi için Beden Algısı Ölçeği (BAÖ) uygulanmıştır.Bulgular: SAB hastalarında sağlıklı kontrollerle karşılaştırıldığında CİB alt paremetrelerinin tamamında düşük skorlar elde edilmiştir (istek, uyarılma, lubrikasyon, orgazm ve tatmin), bu durum istatistiksel olarak anlamlı tespit edilmiştir. Ayrıca hasta grubunda beden algısı puanları kontrol grubundan istatistiksel olarak anlamlı derecede düşük bulunmuştur. Olguların BAÖ puanı ile CİB istek ve orgazm alt boyutu puanı arasında negatif yönde istatistiksel olarak anlamlı ilişki olduğu saptanmıştır.Sonuç: Bu çalışmada Sosyal Anksiyete Bozukluklu (SAB) kadınlar da cinsel işlevlerin bütün aşamalarında sağlıklı kontrollerden daha fazla şikayetleri olduğu tespit edilmiştir. En güçlü bozulma orgazm alanında bulunmuştur. Ayrıca bu kadınlarda beden algısı puanları da kontrollerden oldukça düşük bulunmuştur. Bu sonuç SAB' lu kadınların bedenlerinden memnuniyetsizliğini göstermektedir. Beden memnuniyetsizliği arttıkça CİB' nun istek ve orgazm alanlarında özellikle kötüleşme oluyor gibi görünmektedir.

Evaluation of Body Perception and Sexual Dysfunctions in Patients with Social Anxiety Disorder

Objective: In general, it is known that anxiety disorders affect sexual function negatively, but there are limited studies in this issue. Moreover, according to our literature knowledge, there was no study evaluating the body image perseption and sexual functions together in Social Anxiety Disorder (SAD) patients. In this study, it was aimed to compare female patients with SAD with healthy controls in terms of perception of body image and sexual dysfunction.Method: 30 female patients with a diagnosis of SAD according to DSM-IV TR criterias who has regular sexual life and applied to the outpatient treatment unit of Bozok University Medical Faculty Mental Health and Diseases Department were included in the study. Control group was established with 28 healthy female individuals. The Liebowitz Social Anxiety Scale (LSAS) was used to determine the severity of social anxiety symptoms, the Arizona Sexual Experiences Scale (ASES) was used to assess sexual function, and the Body Cathexis Scale (BCS) was used to assess perception of body image.Results: A statistically significant elevation was found in all of the sexual dysfunction subparameters (desire, arousal, lubrication, orgasm and satisfaction) in SAD patients compared to healthy controls. In addition, the perception of body image scores in the patient group were statistically significantly lower than the control group. There was a statistically significant negative correlation between BSS score and sexual dysfunction request and orgasm subscale scores of the cases. Conclusion: This study demonstrates that women with SAD had more complaints than sexual controls at all stages of sexual functioning. Most prominent deterioration was found in the field of orgasm. As the body dissatisfaction increases, it seems to be especially deteriorating in the areas of the desire and orgasm of the sexual dysfunction.
