KORONER ARTER BAYPAS CERRAHİSİ YAPILAN HASTALARDA ERKEN DÖNEMDE GÖRÜLEN NÖROKOGNİTİF DEĞİŞİKLİKLER İLE İNTRAOPERATİF SEREBRAL OKSİMETRE DEĞERLERİ ARASINDAKİ İLİŞKİNİN ARAŞTIRILMASI
Amaç: Ameliyat sonrası kognitif fonksiyon bozukluğu, kalp ameliyatından sonra sık görülen bir nörolojik komplikasyondur. Tanıda standartlaşmış klinik değerlendirme testlerinin olmaması ve uygulanan testlerin zaman alıcı olması, genellikle bu sorunun göz ardı edilmesine yol açmaktadır. Kognitif fonksiyon bozukluğu postoperatif dönemde hastanede kalış süresini uzatmakta, iş gücü kaybını artırmakta, tedavi maliyetininde de artışına neden olmaktadır. Çalışmanın amacı koroner arter bypass cerrahisi (KABC) yapılan hastaların ameliyat esnasında kaydedilen serebral oksijen saturasyon değerleri ile postoperatif erken dönemde görülen nörokognitif (NK) bozukluklar arasındaki ilişkiyi araştırmaktır. Hasta ve Yöntemler: Çalışmaya merkezimizde 05/09/2014 ile 05/10/2014 tarihleri arasında KABC uygulanan, 40 hasta dahil edilmiştir. KABC yapılan hastaların operasyondan 1 gün önce, postoperatif 3 ve 5. günlerde nörokognitif testler yapılmıştır. Bu testler: Sözel bellek süreçleri testi, mental kontrol konsantrasyon testi, sayı menzili testi ve sözel akıcılık testidir.Serebral oksijen düzeyleri (rSO2) operasyona girişte, operasyon süresince ortalama değeri rSO2, en düşük ve en yüksek serebral oksimetre rSO2 değerleri kaydedilmiştir. Operasyon öncesi nörokognitif test puanları ile postoperatif 3 ve 5. gündeki test puanları arasındaki farklar bulunmuş, bu farklarla serebral oksimetre değerleri arasındaki korelasyonlar araştırılmıştır. Sonuç: Çalışmaya katılan KABC olan tüm hastalarda nörokognitif test skorlarının postoperatif 3. günde operasyon öncesine göre azaldığı görülmüştür. Bununla beraber bu değerler tüm hastalarda postoperatif 5. günde tekrar anlamlı olarak postoperatif 3. güne göre yükselmiştir. Giriş anındaki rSO2 değeri arttıkça ve giriş rSO2 değeri ile intraoperatif ortalama rSO2 değeri arasındaki fark azaldıkça anlık bellek, tanıma ve genel nörokoğnitif test skorları postoperatif test skorlarındaki düşüş azalmaktadır. İntraoperatif maximum rSO2 değeri arttıkça posoperatif 5. gündeki nörokognitif test skorları iyiye doğru gittiği görülmüştür.Yorum: KABC operasyonu sonrası erken dönemde nörokognitif fonksiyonlar bozulmaktadır; ancak bu bozulma postoperatif geçen süre ile düzelme göstermektedir. Serebral saturasyonu iyi olan ve operasyon esnasında serebral saturasyonu daha iyi korunan hastalarda nörokognitif disfonksiyon daha az olmaktadır.
Relationship Between the Intraoperative Cerebral Oxymeter Values and Neurocognitive Dysfunction Among the Patients Undergoing Coronary Arter Bypass Graft in the Early Period
Objective: The postoperative cognitive dysfunction is one of the common neurologic complications after cardiac surgery. The lack of standardized clinical evaluation tests in diagnosis and the fact that the applied tests are time-consuming usually makes it overlooked. Cognitive dysfunction in the postoperative period extends the duration of hospital stay, loss of labor force increases, the hospital costs. The aim of the study is to investigate the relationship between cerebral oxygen saturation values seen in the early postoperative period and neurocognitive disorders in the patients undergoing CABG.Patients and Methods: 240 patients undergoing CABG in our clinic (Pendik Marmara University Training and Research Hospital, Cardiovascular Surgery ward) between 05/09/2014 and 10/05/2014 were included. Neurocognitive tests were applied to the patients one day before the operation, postoperative 3rd and 5th days. These tests: Verbal memory test processes, mental control, concentration test, test of the number range and verbal fluency test. Before the operation the initial cerebral oxygen level (rSO2), during the operation the lowest, the average and the highest rSO2 values were recorded. The differences between the neurocognitive test scores of preoperative and postoperative 3rd and 5th days were calculated and the correlation of neurocognitive test scores and cerebral oxygen levels were investigated.Results: On the postoperative 3rd day the neurocognitive test scores were lower in comparison with the preoperative values. And on the postoperative 5th day these values were higher in comparison with the postoperative 3rd day. As the initial rSO2 value increases and the deficiency between the inital rSO2 and intraoperative mean rSO2 values decreases, the neurocognitive test scores on the postoperative 3rd and 5th days increase. It was also observed that as the intraoperative maximum rSO2 value increases, neurocognitive test scores on the postoperative 5th day change for the better.Conclusion: Neurocognitive functions can be affected badly after CABG in the early period. But this stuation gets better on the postoperative period. Neurocognitive dysfunction is less seen in patients having good preoperative and intraoperative cerebral saturation values.
