Travma hastalarında antibiyotik kullanımı
Profilaktik antibiyotik yaraya bulaşan canlı bakterilerin sayısında kabul edilebilir bir azalma sağlamak ve olabilecek kontaminasyonu normal konakçı mekanizmalarının karşı koyabileceği düzeye indirmek amacı ile yapılır. Cerrahide profilaksi cerrahi girişimden hemen önce uygulanmalı ve mümkün olduğunca kısa süreli olmalıdır. Ayrıca geniş spektrumlu antibiyotikler tercih edilmemelidir. Ancak travma hastalarında bu ilkelerin tamamına uymak mümkün değildir. Zira travma zamanı bilinemeyeceğinden travma öncesinden antibiyotiğe başlama imkanı da yoktur. Avrupa Cerrahi İnfeksiyon Derneği infeksiyon riskinin % S 'in üzerinde olduğu durumlarda antibiyotik profilaksisi önermektedir. Temiz-kontamine ve kontamine yaralarda infeksiyon riski % 5'in üzerindedir. Travma sonucu gelişen yaralanmalar genellikle bu sınıfa girmekte olup bu hastaların çoğunda antibiyotik profilaksisi gereklidir.
Antibiotic use in trauma patients
Prophylaxis is applied to reduce the number of living bacteria to an acceptable and resistable level for the normal host defense mechanisms in a wound possibly contaminated. In surgery, prophylaxis should be administered just before the surgical procedure and should be continued as short as possible, Broad spectrum antibiotics should not be preferred. It is not possible to keep all of these criteria in all the times in patients with trauma. We can not know when the trauma will take place, so we can not administer the antibiotic previously. European Surgical Infection Society suggests the use of antibiotic prophylaxis if the infection risk is higher than 5 %. This risk is higher than 5 % in clean-contaminated and contaminated types of wounds. Since the wounds due to trauma are usually these types, antibiotic prophylaxis is necessary in most of wounded patients.
