Toplum kökenli akut ateşli hastaya klinik yaklaşım
Ateşinin ilk 7 günü içinde başvuran bir olgu akut ateşili hasta olarak kabul edilebilir. Akut nedeni bilinmeyen ateş ise ateşinin ilk 7 günü içinde başvuran bir olguda ayrıntılı anamnez ve fizik muayene bulgularına göre ateşin nedeninin saptanamadığı durum olarak tanımlanmaktadır. Akut nedeni bilinmeyen ateşte hospitalizasyon endikasyonları; 1- > 50 yaş (özellikle > 65 yaş), 2-eşlik eden hastalık (DM, immündüşkünlük, alkolizm), 3-lökositoz (a 15,000 /mm3), 4-çomakların artışı (sola kayma, > 1500 /mm3), 5-nötropeni ( 41°C)'dir. Toplum kökenli akut ateşi olan bir hastadan; pürülan menenjit, ağır sepsis veya akut endokardit olmasın diye korkarız. Bu 3 'lü hastalık saatler içinde öldürebilir ve tedavisiz mortaliteleri % 100'dür. Bu nedenle akut ateşi olan her olguda bu 3 hastalık mutlaka akla getirilmeli ve olup olmadığı aranmalıdır. Poliklinik dosyasına veya konsültasyon formuna hastanın bu yönden değerlendirildiği not olarak düşülmelidir.
A clinical approach to the patient with community acquired acute fever
A patient admitted with a fever of a recent 7 days is accepted to have an acute fever. Acute fever of unknown origin is defined as the failure to find the etiology of the fever in a patient admitted with fever of<7 days after a thorough history and physical examination. Hospitalization for acute fever ofunkown origin is needed in the cases of: 1- those who are >50 year-old (especially > 65 year-old), 2-underlying disorders (DM, immunocompromised, alcoholism), 3-leukocytosis (> 15,000 /mm3), 4-an increase in band forms of neutrophils (a left shift, ^1500 /mm3), 5-neutropenia (<1000 I mm3), 6-accelerated erythrocyte sedimentation rate (> 30 mm/hour), 7- septic or toxic appearence of the patient (a quite ill patient), and 8-hyperpyrexia (>41 °C). Purulent meningitis, severe sepsis, and acute endocarditis are the main considerations in a patient with acute fever due to a community-acquired illness. All these three infections may lead to death within hours and without specific treatment mortality is 100 %. For this reason, these three disorders should be considered and sought in every patients with acute fever. A note describing the absence of these disorders should be written to the outpatient file or consultation form.
