Ateşli çocuklarda ketoprofen ve parasetamolün ateş düşürücü etkilerinin karşılaştırılması

Çocuk Acil Polikliniği’ne başvuran 1-14 yaş arası ve infeksiyona bağlı en az 24 saattir ateşi olan (aksiller >38°C), antipiretik müdahaleyi gerektiren ancak değerlendirme süresince antibiyotik tedavisi verilmeyen 35 olgu incelenmiştir. Ortalama ateşleri 39.1±0.6°C olan bu olgulara randomize olarak 15 mg/kg parasetamol (n=18, grup 1) ve 0.5 mg/kg ketoprofen (n=17, grup 2) uygulanmıştır. Olguların 30., 60. ve 120. dakika ateş değerleri karşılaştırılmış, yan etki profili ve tat uyumu açısından ise olgular 48. saatte tekrar ayrıntılı bir fizik muayene ve anamnezle değerlendirilmiştir. Olguların 30.dakika değerleri: grup 1 ve 2’de sırasıyla 38.3±0.8°C ve 38.5±0.6°C (p>0.05), 60.dakika değerleri: 37.6±0.8°C ve 37.6±0.7°C (p>0.05), 120.dakika değerleri: 37.1±0.6°C ve 37.2±0.4°C (p>0.05) olarak bulunmuştur. Her iki grupta da 30., 60., 120. dakika değerlerinde başvuruya göre anlamlı düşüklük saptanmıştır (p0.05). Sonuçta her 2 grupta da ateş düşürücü etki ve yan etkiler benzer bulunmuştur. Ketoprofen parasetamole alternatif olarak etkili ve güvenilir bir ajan olarak değerlendirilmiştir.

Comparison of antipyretic effects of ketoprofen and paracetamol in febrile children

In this study, we evaluated 35 children aged 1 year to 14 years and admitted to pediatric emergency department with fever (axillary >38°C) at least 24 hours because of an infectious disease. None of the patients had taken antibiotherapy. Mean temperature value of these patients was 39.1±0.6°C and they were treated randomly with 15 mg/kg/dose oral paracetamol (n=18, group 1) or 0.5 mg/kg/dose oral ketoprofen (n=17, group 2). We evaluated the body temperature of the subjects at 30., 60. and 120. minutes of the treatment. On the other hand, side effect profile and compliance for these drugs were also tested in the second visit of these cases at 48. hours. Body temperature measurements at 30. minute for group 1 and 2 were 38.3±0.8°C and 38.5±0.6°C (p>0.05), at 60. minute were 37.6±0.8°C and 37.6±0.7°C (p>0.05), and at 120. minute were 37.1±0.6°C and 37.2±0.4°C (p>0.05), respectively. For both groups, we found that the temperature values at 30., 60. and 120. minutes were lower than those measured at the first contact (p<0.001). Vomiting was observed in one patient after the paracetamol treatment, but this was not related to paracetamol because the case was diagnosed as acute gastroenteritis. Taste compliance for group 1 and group 2 was 61 % and 82 % (p>0.05), respectively. In conclusion; antipyretic effect and side effects were similar for both groups. Therefore, ketoprofen appeared effective and safe, so it may be an alternative to paracetamol.
