Metoklopramid Kullanımına Bağlı Gelişen Akut Distoni Olgusu

Özet: Metoklopramid Kullanımına Bağlı Gelişen Akut Distoni OlgusuMetoklopramid, dopamin reseptör antagonistidir. Antiemetik olarak oldukça sık kullanılan bir ilaçtır. Kan beyin engelini aşarak, parkinsonizm, malign nöroleptik sendrom, akatizi, tardif diskinezi ve akut distonik reaksiyon gibi ekstrapiramidal semptomlara sebep olur. Olgumuz intihar amaçlı iki adet 200 mg asetilsistein kapsül, iki adet 500mg asetaminofen + 50 mg kafein ile iki adet 10 mg metoklopramid tablet aldıktan yaklaşık 1-2 saat sonra boynunda istemsiz kasılmalar gelişmesi üzerine acil servisimize başvuran 16 yaşında bayan bir hastadır.

Acute Dystonia Case Due to Metoclopramid Use

Metoclopramid is the dopamine receptor antagonist. It has been used widely as antiemetic agent. This drug enters the blood-brain barrier and causes extrapyramidal symptoms such as Parkinsonism, malign neuroleptic syndrome, akathisia, tardive dyskinesia and acute dystonic reaction. Our case is a 16-year-old female, who took two tablets of 200 mg acetylcystein, two tablets of 500 mg acetaminofen plus 50 mg caffein, and two tablets of 10 mg metoclopramid for the purpose of suicide and applied to our emergency service with the involuntary spasm in the neck after few hours.
