Ağır psikiyatrik bozukluklar, depresyon ve sigara bırakma

Son yıllarda ülkemizde sigarayı bırakma konusunda çok ciddi kampanyalar düzenlenmekte, sağlık çalışanları bu konuda bilinçlendirilmekte ve devlet eliyle bu alana özel tedavi hizmetleri verilmektedir. Ancak ciddi psikiyatrik bozukluğu olanlar arasında sigara içme çok yaygın olmasına ve ciddi bir mortalite ve morbidite nedeni olmasına rağmen psikiyatri uzmanları, psikiyatri hastalarında sigarayı bıraktırma konusunda çok gayretli davranmamakta- dır. Oysa son yıllarda yapılan tedavi çalışmaları göstermiştir ki, ciddi psikiyatrik bozukluğu olan ve klasik bilgilere göre sigarayı bırakmaları çok zor olarak görülen kişilerde de sigarayı bırakma tedavilerinin bazı değişiklikler yapı- larak uygulanmasıyla başarılı sonuçlar alınabilmektedir. Son yıllarda sigarayı bırakma amaçlı ilaç tedavilerinde de gelişmeler olmuştur. Ancak bu alanda kullanılan ilaçların ciddi nöropsikiyatrik yan etkileri olabilmektedir. Her hastada yarar ve risk dengesi gözetilerek uygun tedavi seçeneğinin belirlenmesi gereklidir.

Severe mental disorders, depression and smoking cessation

In recent years, large campaigns have been organized for smoking cessation, health professionals have been educated about this topic and special treatment programs have been developed by the government for smokers. Although cigarette smoking is very frequent among patients with severe mental illness and it’s a cause of morbi- dity and mortality, psychiatrists are not very eager for smoking cessation among psychiatric patients. However, in recent years, treatment studies have shown that with some modifications, conventional smoking cessation ap- proaches may be quite beneficial among these severe mentally ill patients who are classically known to be fail- ures in such treatments. In recent years, there have been developments in pharmacological approaches for smoking cessation, but neuropsychiatric side effects of these medications may be serious. Risk and benefit ratio for each patient should be taken into account while making a treatment plan.
