İşlem Maliyetleri Teorisi: Sağlık Hizmetleri Sunumunda Sözleşme mi? Yoksa Örgüt İçi Yapılanma mı?

Temelde kamu kaynaklı bir endüstri olma özelliği taşıyan Türk sağlık sistemi, yaklaşık on yıldır, özel sektör kaynaklarının artan kullanımı yönünde bir eğilim sergilemektedir. Bu durum özellikle ev idaresi hizmetleri ve bazı klinik hizmetlerin sunumunda göze çarpmaktadır. Örgüt teorileri ne zaman-neden-nasıl sorularını cevaplandırarak, örgütlerin davranış ve yapılanma şekillerini anlama, açıklama ve tahmin etmekte önemli katkı sağlarlar. Bu makale iki amaca hizmet etmektedir. Birincisi, sağlık bakım hizmeti sunan kuruluşların hizmet işlemlerini verimli şekilde gerçekleştirebilecekleri mekanizmaların seçimine ışık tutabilecek bir çerçeve sunan İşlem Maliyetleri Teorisi hakkında temel bilgileri aktarmaktır. İkincisi, İşlem Maliyetleri Teorisinin sunduğu bilgi dahilinde, özellikle diyaliz tedavisinin sunumu ile ilgili işlemlerin gerçekleştirilmesinde yaşanan gelişmeleri genel olarak yorumlamaktır.

Transaction Costs Theory: Contraction or Organizational Reform in the Supply of Health-Services?

It is clearly observed that the private sector has increasingly gone into the health services supply market especially for the last ten years in Turkey. Turkish health care system was mainly based on the public funded supply but this situation has changed with a movement towards increasing utilization of the services provided by the private health care sector. This gradual shift has been observed mainly in the supply of housekeeping services and some kinds of clinical services. By taking into consideration of the important contribution ,of the organization theories to grasping, explaining and to estimate of the behavioral and structural forms of the organizations, the article has two aims: one is to provide a basic understanding for the Transaction Costs Theory which offers a useful framework for health care providers in their decisions on selection mechanisms to carry out efficient transactions. Second, is to draw a comment about the recent developments that especially taken place in dialysis care-related service transactions.
