Gece Yeme Sendromunun Psikopatoloji Çerçevesinde İncelenmesi

Gece yeme sendromu (GYS) ilk kez Stunkard (1955) tarafından tedaviye dirençli obezite hastalarındatanımlanmıştır. Tanı ölçütleri incelendiğinde, sabahları ortaya çıkan anoreksinin; kahvaltının atlanmasıveya oldukça az miktarda yenmesinin (örneğin kahve veya meyve suyu), akșam hiperfajisinin; günlüktoplam kalorinin en az %25’inin akșam yemeğinden sonra alınmasının, insomnia veya uykusuzluğun isehaftada 3 veya daha fazla olmasının hakim olduğu bir klinik tablo görülmektedir. GYS bulunan bireylerdepsikososyal stres faktörlerine sıkça rastlandığı bilinmekte ve bu stresörlerin yeme atakları ile ilişkili olduğu savunulmaktadır. Genellikle bireyler kendilerini üzgün hissettiklerinde ve negatif duygu durumlarınasahip olduklarında besin alımını, sorunla başa çıkma aracı olarak kullanmaktadırlar. Birçok araştırmacı,stresle baş etme veya problem çözme hususundaki yetersizliklerin yeme bozukluklarına sebep olabileceğini belirtmektedir. Bu doğrultuda, yeme bozukluklarının problemlerle baş etmede zorluk çekme ile bağlantılı olduğu düşünülebilir. Her birey yaşadığı sıkıntılar karşısında farklı tepkiler vermektedir. Birçokkişinin üzgün olduğunda veya kendini mutsuz hissettiğinde rahatlatmak için yemek yemeye başvurduğudikkat çekmektedir. Yeme bozukluğuna sahip olan kişilerin birçoğu depresif belirtiler göstermektedir.Hissedilen mutsuzlukla başa çıkmaya ve stres düzeyini azaltmaya yönelik çabalar, kontrolsüzce ve aşırımiktarda yemek yemeye başlamanın nedenleri arasında yer almaktadır. Bu çalışmada, gece yeme sendromunun psikopatoloji çerçevesinde incelenmesi amaçlanmıştır.

Investigation of Night Eating Syndrome within the Framework of Psychopatology

Night eating syndrome (NES) was first described by Stunkard (1955) in treatment-resistant obesity patients. When diagnostic criteria are examined, there is a clinical picture that is characterized by anorexia in the morning; skipping breakfast or eating negligible amounts (eg coffee or juice), evening hyperphagia; consuming at least 25% of total daily calories after dinner and, three or more bouts of insomnia or sleeplessness during a week. Psychosocial stress factors are very common in people with NES and these are associated with eating attacks. Generally, when individuals feel sad themselves and have negative emotions, they use their food intake as a means of coping with problems. Many researchers suggest that inadequacy in coping with stress or in problem solving can lead to eating disorders. Consequently, it can be thought that eating disorders are associated with difficulty in coping with problems. Each individual reacts differently to the problems he / she experiences. It is noteworthy that many people resort to eating when they feel sad or unhappy. Many people with eating disorders show depressive symptoms. Efforts to cope with unhappiness and to reduce stress are among the reasons for beginning uncontrolled and excessive eating.
