Crohn hastalığı seyrinde görülen Pemfigus foliaseus olgusu

İnflamatuvar barsak hastalıkları seyrinde hastalığın aktivitesi ile ilişkili olabilen ya da olmayan çeşitli barsak dışı tutulumlar görülebilmektedir. Bu lezyonların bir kısmı kesin olarak bilinmekte, bir kısmı ise vaka tak- dimi şeklinde literatürde sunulmaktadır ve bunların görülme oranları ile ilgili rakamlar değişiklik göstermektedir. Otoimmün büllöz hastalıklar da inflamatuvar barsak hastalıkları seyrinde nadir olarak görülebilen, bir kısmının birlikteliği net olarak ispatlanmamış hastalıklardır. Daha önce- den Crohn hastalığı tanısı olan ve medikal tedavi ile remisyonda seyre- derken, enterovezikal fistül nedeniyle opere edilen ve sonrasında vücu- dunda büllöz lezyonları ortaya çıkan hasta hastanemiz gastroenteroloji servisine yatırıldı. Yapılan tetkikler sonrasında Pemfigus foliaseus olarak tanı alan olgu uygulanan tedaviye cevap verdi ve lezyonları geriledi. Literatürde daha önce gösterilmeyen Crohn hastalığı seyrinde görülen Pemfigus foliaseus vakası bizim olgumuzda beraber görülmüştür.

A case report: A pemphigus foliaceus case seen in the course of Crohns disease

Various extraintestinal findings, which may or may not be associated with disease activity, can be seen in the course of inflammatory bowel diseases. Some of these lesions are absolutely known while a few are reported in the literature as case reports; the exact ratio of the lesions vary. Autoimmune bullous diseases are rarely seen in the course of inflammatory bowel diseases. Herein, we report on a case of a Crohns diesease patient, in remission using oral mesalazine, who was operated on for an enterovezical fistula. Bullous lesions arose after the operation, noted by an intern on our gastroenterology service. After investigation, he diagnosed the lesions as pemphigus foliaceus. The patient respond- ed to treatment and the lesions ceased. A pemphigus foliaceus case together with crohns disease, as reported in our case, has not yet been described in the literature.
