Boyun ve yüz bölgesine uygulanan minör travma sonucu oluşan post-travmatik pnömomediastinum olgusu
Pnömomediastinum; medias- ten içerisinde hava bulunması olarak tanımlanır ve spontan, iatrojenik, travmatik olarak 3 gruba ayrılır. Olgumuz; başın- dan, yüzünden ve boynundan künt travmaya maruz kalmış 22 yaşında bir kadındır. Yapılan fizik muayeneleri, kan testleri ve bilgisayarlı tomografileri ta- ramaları sonucunda; özefagus etrafında ve ön mediastende minimal multiple hava imajla- rı (pnömomediastinum) görül- müştür. Pnömo/hemotoraks ya da kosta fraktürü izlenmemiş- tir. Klinikte oksijen inhalasyon tedavisi ve 24 saatlik grafileri ile takip ve diğer tedavileri ya- pılmıştır. Olgu, adli tıbbi açıdankişinin yaşamını tehlikeye so- kan bir durum olması ve baş- kaca eşlik eden bir patoloji ol- madan pnömomediastinumun az rastlanan bir durum olması nedeniyle literatür ile karşılaş- tırılarak tartışılmıştır.
The post-traumatıc pneumomedıastınum case occurred as a result of mınor trauma to neck and face
The pneumomediastinum is de- fined as the presence of air in the mediastinum and divided into three groups as spontaneous, iat- rogenic, traumatic. Our case is a 22 years old woman exposed to blunt trauma through her head, face and neck. As a result of the performed physical examinations, blood tests and computed tomog- raphy scans; minimal multiple air collections (pneumomedias- tenium) were observed around the esophagus and in the ante- rior mediastinum. There was no pneumo/hemothorax or rib frac- ture observed. Oxygen inhalation therapy, and 24-hour follow-up with X-rays, and other treatments were performed. The case has been discussed through compa- ring with the literatüre since it s alife-threatening condition for the nvidual from forensic medical point of view and since pneumo- mediastinum without any other accompanying pathology is a ra- rely seen condition.
