Tersiyer Bir Merkezde Polihidramnios Olgularının Retrospektif Değerlendirilmesi
Amaç: Kliniğimizde tanı konup, doğumu gerçekleştirilen polihidramnios olguların fetal ve maternal özellikler açısından literatür eşliğinde değerlendirmek Metaryal-Metod: : 4 yıllık sürede doğum yapan 149 polihidramnioslu gebe, retrospektif olarak değerlendirildi. Hastalar; anne yaşları, gebelik, doğum sayıları, doğum şekilleri, sezeryan endikasyonları, doğumdaki gebelik haftaları, prematürite oranları, bebeklerin 1. ve 5. dakika APGAR skorları, doğum ağırlıkları, polihidramnios etyolojisi açısından retrospektif olarak incelendi. Bulgular: : 13624 hastadan 149 tanesinde polihidramnios(%1.09) mevcuttu. Olguların ortalama yaşı 31.32±6.56, gebelik sayıları 5.49±3.03, doğum sayıları 4.01±2.78 olarak bulundu. Ortalama gebelik haftası 34.06±4.16 ,ortalama doğum ağırlıkları 2298±873 gram idi. Perinatal mortalite oranı %32.21 (n=48) olarak izlendi. En sık etyolojik nedenler sırası ile idyopatik %57.72 (n=86), fetal anomaliler %25.50 (n=38) gestasyonel diabet %11,41 (n=17) idi. En sık fetal anomali santral sinir sistemi anomalileri %16.10 (n=24) olup en sık anensefali %7.38 (n=11) idi. Olguların %1,34ünde (n=2) non immün hidrops fetalis, %4.02sinde (n=6) immün hidrops fetalis izlendi. Doğum şekli oranları sırasıyla sezaryen (%47.65), normal vajinal yol (%52.35) idi Sonuçlar: Antenatal takibin yaygın ve programlı yapılması; polihidramniyozun etyolojisinin erken tespiti ile önlenebilir nedenlerine daha erken mü- dahele imkanı verip maternal , fetal morbidite ve mortaliteyi azaltacaktır. Hastaların tersiyer merkezlere erken refere edilmesi erken müdahele imkanı verecektir.
Retrospective Analysis of Polyhdramniotic Cases in a Tertiary Center
Aim: To evaluate our polyhydramnios cases regarding to fetal and maternal factors retrospectively. Material and methods: 149 polyhdramniotic pregnancies throughout 4 years were evaluated and maternal age, parity, mode of delivery, indication for cesarean section, delivery week, prematurity, 1st and 5th minute APGAR score, birth weight, end polyhydramnios etiology were recorded. Results: : Polihydramnios rate was %1.09 (149/13624).Mean Maternal age 31.32±6.56, gravidity 5.49±3.03, parity 4.01±2.78, mean gestational week 34.06±4.16 , mean birth weight 2298±873 grams, perinatal mortality rate %32.21 (n=48). Etiological factors were idiopathic %57.72 (n=86), fetal anomalies %25.50 (n=38), and gestational diabetes %11, 41 (n=17), respectively. Most frequent fetal anomaly was central nerve system anomalies %16.10 (n=24) with anencephaly as %7.38 (n=11). %1, 34 (n=2) had non immune hydrops fetalis, whereas %4.0 (n=6) had immune hydrops Fetalis. Mode of deliveries were cesarean (%47.65) and vaginal (%52.35), respectively. Conclusion: : A programmed antenatal surveillance could result in early detection of etiological factors. Patients should be referred to tertiary centers to facilitate early interventions.
