Metastatik mide malign melanomu
Malign melanom primer olarak en sık ciltte lokalize olan agresif seyirli nadir bir tümördür. Nadiren gastrointestinal sistemde primer bir tümör olarak da saptanabilir. Gastrointestinal sisteme en sık metastaz yapan malignensilerden biridir ve özellikle mideye en sık metastaz yapan tümördür Gastrointestinal sistem metastazı primer tanı sırasında saptanabilir ya da yıllar sonra rekürrens belirtisi olarak ortaya çıkabilir. Bazı vakalarda primer bir lezyon saptanamayabilir. Gastrointestinal sistemde saptanan melanomun primer mi yoksa metastatik mi olduğu konusunda ayırım yapılması güç olabilir. Metastatik gastrointestinal malign melanomda prognoz genellikle kötüdür. Bu yazıda asit ve karaciğer metastazları nedeniyle araştırılırken, özofagogastroduodenoskopide saptanan ve histopatolojik olarak tanısı doğrulanan metastatik mide malign melanomu vakasını bildiriyoruz.
Metastatic malignant melanoma of the stomach
Malignant melanoma is an agressive and rare tumor. Most common primary localisation is skin. Rarely it can be detected as a primary tumor in the gastrointestinal system. It is one of the most common malignancies associated with metastatic disease of the gastrointestinal tract especially of the stomach. Metastases to the GI tract can present both at the time of primary diagnosis or several years later as the first sign of recurrence. Primary lesion may not be detected in some cases. Distinguishing between a primary gastrointestinal mucosal melanoma and a melanoma metastatic to the gastrointestinal tract may be difficult. Prognosis is generally poor in metastatic gastrointestinal melanoma. We report here a case of metastatic malignant melanoma of stomach, which detected on esophagogastroduodenoscopy and confirmed by histopathologic examination, in a patient with ascites and liver metastases.
