Persistent fifth aortic arch with systemic-to- pulmonary arterial connection and pulmonary atresia in a neonate
Persistan beşinci aortikark (PBAA) çıkan aortanın distal bölümünden ayrılan gerçek arkus aortanın altından seyrederek sol subklaviyan arter ve duktus arteriyosus seviyesine uzanan bir arterdir. Persistan beşinci aortik ark ilk defa 1969 yılında tanımlandı. Bu çift lümenli aortik ark ya sistemik-sistemik ya da sistemik-pulmoner bağlantılıdır. Ekokardiyografi ile PBAA’dan şüphelenildiğinde tanıyı doğrulamak için kardiyovasküler anjiyografi ve/veya manyetik rezonans anjiyografi çekilmelidir. Semptomatik vakaların nadir olması ve vakanın karmaş ıklığı nedeni ile sistemik-pulmoner bağlantılı tip PBAA’sı olan bir günlük erkek yenidoğan bebek sunulmuştur.

Persistent fifth aortic arch (PFAA) is an artery originating from distal ascending aorta and then extends beneath the true aortic arch to the level of left subclavian artery and ductus arteriosus. Persistent fifth aortic arch was firstly described in 1969. This double lumen aortic arch is either systemic-to-systemic or systemic-to-pulmonary connection. We should perform the conventional cardiovascular angiography and/or magnetic resonance imaging angiography to be sure about diagnosis, when we suspect presence of PFAA with echocardiography. Because of the rarity of symptomatic cases and its complexity, we describe a one-day-old male newborn baby with systemic-to-pulmonary type PFAA.
