Vertebral kemik metastazı ile tanısı konan papiller tiroit karsinom olgusu

Papiller tiroit kanseri, en sık görülen tiroit malignitesi olup, iyi prognoza sahiptir. Uzun süre sessiz kalan bu tümörler daha çok bölgesel lenf nodlarına metastaz yapma eğilimindedir. Akciğer ve kemik gibi uzak organ metastazı nadir görülmekte ve uzak metastaz ile tanı alan olgulara literatürde oldukça az sayıda rastlanmaktadır. Bu olgu sunumunda nörolojik yakınmaları nedeni ile yapılan tetkiklerinde, torakal vertebralarda tiroit karsinom metastazı saptanan 56 yaşında erkek hastada uygulanan kombine tedavi yaklaşımı ve klinik seyir sunulmaktadır. Ayrıca bu olgu dolayısıyla literatür verileri gözden geçirilerek, tanı anında kemik metastazı bulunan iyi differansiye tiroit kanserlerinin, klinik izlem sonuçları gözden geçirilmektedir.

A case of papillary thyroid carcinoma diagnosed with vertebral bone metastasis

Papillary carcinoma of the thyroid gland is the most common thyroid malignancy with a good prognosis. These tumors remain asymptomatic for a long time and mostly tend to make metastasis to regional lymph nodes. The involvement of distant organs like lungs and bones is rare and the occurrence of distant metastasis at initial presentation is also unusual. In this report, the clinical course of a patient initially diagnosed with vertebral metastasis of papillary thyroid carcinoma who underwent total thyroidectomy combined with additional therapies is presented. Additionally, the literature data obtained on the clinical course of patients presented with metastatic disease at the time of diagnosis are reviewed.
