Amaç: Büyük arter transpozisyonunda (D-TGA) arteriyel switch ameliyatları günümüzde kabul edilen tedavi yöntemidir. Operasyonda koroner translokasyonların sorunsuz olması mortaliteyi düşürmektedir. Materyal ve Metod: Anabilim Dalımızda 1997 yılından beri 40 D-TGA olgusuna arteriyel switch ameliyatı yapıldı. Yenidoğan dönemini geçirmiş intakt ventriküler septumlu 7 olguya two stage yaklaşım ile arteriyel switch prosedürü uygulandı. VSD + D-TGA'lı 8 olguya ise VSD kapatılması ve arteriyel switch operasyonu yapıldı. 26 olgu neonatal dönemde, 1 olgu 12. saatte, 4 olgu 24. saatte opere edilmiştir. Bulgular: Başlangıç deneyimimizde 2 olgu peroperatif kaybedildi. 4 olgu postoperatif uzayan entübasyon, sepsis ve düşük kardiyak debi gibi nedenler ile kaybedildi. Son 15 vakadan sadece 1 olgu kaybedilmiştir. Sonuç: Arteriyel switch operasyonlarının cerrahi deneyimin artmasıyla düşük mortalite ile yapılabileceğini düşünüyoruz.
Background: Arterial switch operations are the current treatment of choice in transposition of the great arteries (D-TGA). Coronary translocation is the most important factor affecting the morbidity and mortality. Methods: From January 1997, 40 cases of D-TGA underwent arterial switch operation. Seven patients with intact ventricular septum who could be diagnosed beyond the neonatal period underwent a two stage arterial switch operation. 8 patients with complex D-TGA associated with a VSD underwent arterial switch and VSD closure operation. 26 cases were neonate. Results: 2 patients died peroperatively. 4 patients died postoperatively because of prolonged intubation, sepsis and low cardiac output. In the last 15 cases, there was only 1 patients died. Conclusions: We are believing that with increasing experience arterial switch operation can be performed with acceptable mortality ">
