"Kırılgan" yaşlı kavramının halk sağlığı yönü

Kırılgan yaşlı yaşlanan dünyada geriatri alanında çalışan hekimleri yakından ilgilendiren bir konudur. Klinik bir sendrom olarak da kabul edilen bu durumun tanısı vücut ağırlığında istemsiz kayıp, kişinin kendisini tükenmiş hissetmesi, bedensel etkinlikte azalma, yürüme hızında azalma ve güçsüz hissetme gibi kriterler üzerinden konulabilmektedir. Dünya’da yaşın da ilerlemesiyle birlikte %3-23 arasında değişen bir görülme sıklığı tanımlanmaktadır. Tanı için geçerlik ve güvenirliği ülkemizde de yapılmış ölçeklere gereksinim bulunmaktadır. Tedavi seçenekleri klinisyenler tarafından yapılması uygun olan bu durumun önlenmesi /geciktirilmesi için sağlığın korunması ve geliştirilmesi yaklaşımlarına gereksinim bulunmaktadır. Bu yazıda, klinik bir sendrom olan kırılganlık halk sağlığı bakış açısıyla değerlendirilmiştir.

Public health aspect of "frail" elderly concern

Frail elderly is a theme which the medical doctors working in geriatrics related issues are are concerned with. Diagnosis of frailty which is also accepted as a clinical syndrome can be done via diagnostic criteria including unintended weight loss, feeling exhausted, decreasing physical activity, slowing, and weakness. The frequency of frailty in the world changes between 3-23% as age increases. There is a need for a national validated frailty index for diagnosis. In order to prevent or delay frailty, prevention of health and health promotion approach should be applied besides the curative alternatives recommended by the clinicians. In this paper, frailty which is already a clinic syndrome has been assessed in public health perspective.
