Molding of the regenerate in reconstruction of the temporomandibular joint by distraction osteogenesis: A case report

Distarksiyon osteogenezisinde rejeneratn ekillendirilmesi literatürde kabul edilen bir prosedürdür. Bu ilem genellikle konsolidasyon periodunda elastik traksiyon ile uygulanmaktadr. Bu olgu sunumunda, temporomandibular eklemin transport distraksiyon osteogenezisi ile rekonstrüksyonu srasnda akut ekillendirme ilemi uygulanm tr. Aktivasyon periodunun 10. gününde transport segmentin uygun olmayan vektörde olduu fark edilmi ve segmente manuel olarak yaklak 150 kadar ekillendirme ilemi uygulanm tr. ki yllk takip sonucunda olguda iyi bir temporomandibular eklem fonksiyonu ve kemik oluumu izlenmitir.

Molding of the regenerate in distraction osteogenesis has been accepted as an applicable procedure. The method has been applied gradually more often in the consolidation period by elastic traction. In this case report, we applied acute molding of the regenerate manually during the transport distraction osteogenesis for reconstruction of temporomandibular joint. On the 10th day of the activation period, we noticed that the vector of the segment had not been on the correct direction. Immediately, we positioned the vector of the device on about 15 degree by a careful digital remodelling. Two years after surgery there was a good temporomandibular joint function with good bone healing.
