COVİD-19 İLE ENFEKTE VE FOURNİER GANGRENİ OLAN BİR HASTANIN HEMŞİRELİK BAKIMI: OLGU SUNUMU

Fournier Gangreni, hızlı ilerleyen ve yüksek mortalite oranına sahip nekrotizan fasit ile karakterize bir enfeksiyon hastalığıdır. Anorektal bölge ve genital cilt alanlarında karşılaşılan bu hastalıkta acil cerrahi tedavi yaklaşımı nekrotik dokuların debridmanı olmaktadır. Debridman sonrası hastanın hemşirelik bakımı kritik öneme sahip olup eşlik eden hastalıkların olması durumunda bakım ve tedavi süreci, hasta ve sağlık ekibi için güç bir hal almaktadır. Bu süreçte cerrahi hemşiresinin rol ve sorumlulukları belirgin bir şekilde ön plana çıkmaktadır. Bu olgu çalışmasında amaç Fournier Gangreni olup COVİD-19’ un eşlik ettiği bir hastanın postoperatif hemşirelik bakımını bir model eşliğinde sunarak hemşirelik bilgisinin artmasını sağlamaktır. Veriler, Roper, Logan, Tierney’in Günlük Yaşam Aktiviteleri Modeli kullanılarak hastaya ait perioperatif süreçteki bilgilerinden yararlanarak elde edilmiştir. Hasta için NANDA-I Hemşirelik Tanıları, Hemşirelik Sonuçları Sınıflandırması ve Hemşirelik Müdahaleleri Sınıflandırması sistemleri ile hemşirelik süreci planlanmıştır. Olguda problem odaklı hemşirelik tanıları ile birlikte risk odaklı hemşirelik tanıları belirlenmiştir ve olgunun hemşirelik bakımı sağlanmıştır. Bu olguda Fournier Gangreni olan bir hastanın postoperatif bakımının COVID-19 nedeniyle güç bir hal aldığı görülmüştür. Bu çalışmanın izolasyon yoğun bakım ünitesinde yatmakta olan hastaların postoperatif bakım gereksiniminin planlı hemşirelik bakımı ile yönetilmesi ve sonuçlarının izlenmesine örnek oluşturacağı, benzer koşullarda hasta bakımı sağlayan hemşirelere yol gösterici olacağı değerlendirilmektedir.

NURSING CARE OF A PATIENT INFECTED WITH COVID-19 AND WITH FOURNIER GANGRENE: CASE REPORT

Fournier's Gangrene is an infectious disease characterized rapidly progressive necrotizing fasciitis with high mortality rate. In this disease, which is encountered in anorectal region and genital skin areas, the immediate surgical treatment approach is debridement of necrotic tissues. Nursing care of the patient after debridement is of critical importance, and in case of comorbidities, the care and treatment process becomes difficult for the patient and the healthcare team. In this process, the roles and responsibilities of the surgical nurse come to the fore. The aim of this case study is to increase the nursing knowledge by presenting the postoperative nursing care of a patient with Fournier's Gangrene accompanied by COVID-19 with a model. The data were obtained by using Roper, Logan, Tierney's Activities of Daily Living Model, using the patient's information in the perioperative process. The nursing process is planned for patient with NANDA-I Nursing Diagnoses, Nursing Outcomes Classification and Nursing Interventions Classification systems. In the case, risk-focused nursing diagnoses were determined together with problem-focused nursing diagnoses and nursing care was provided for the case. In this case, it was observed that postoperative care of a patient with Fournier's Gangrene became difficult due to COVID-19. It is considered that this study will set an example for the management of postoperative care needs of patients in isolation intensive care unit with planned nursing care and monitoring the results, will guide nurses who provide patient care similar conditions.
