Mikrositik anemilerde eritrosit indeksleri

Hafif anemiyle seyreden talessemi minör ve kronik hastalık anemisi ile, hafif düzeydeki demir eksikliği anemisinin birbirinden ayırt edilmesi, özellikle talesseminin yoğun olduğu bölgelerde ve hastanede yatan hastalarda önem kazanmaktadır. Bu çalışmada hafif düzeydeki mikrositik anemili hastalarda eritrosit indekslerinin ayırıcı teşhisdeki rolü araştırıldı. Hb>9.5 gr/dl üzerinde olan 35 demir eksikliği, 20 kronik hastalık anemisi ve 25 talessemi minörlü hastada eritrosit indeksleri incelendi. Fe eksikliğinde Hg düzeyi ile ortalama eritrosit hacmi (MCV) ve ortalama eritrosit hemoglobini (MCH) arasında pozitif korelasyon bulunurken talessemi minörde korelasyon bulunamadı. Talessemi minörde eristrosit sayısı diğer 2 gruptan daha yüksek, MCV ve MCH ise daha düşük bulundu. Demir eksikliği ile kronik hastalık anemisi arasındaki fark ise sadece MCV ve MCH nin demir eksikliğinde daha düşük olmasıdır. Eritrosit dağılım genişliği (RDW) değerleri her üç grupta normalden yüksek olmakla birlikte aralarında istatistiki bir fark bulunmadı.

Erythrocyte indexes in microcytic anemias

Differentiation of mild degree iron deficiency anemia from thalassemia minor and chronic disease anemia is an important issue especially for hospitalized patients and subjects living in the thalassemia-rich areas. In this study, the role of erythrocyte indexes in differential diagnosis of patients with microcytic anemia was investigated. Erythrocyte indexes of 35 iron deficiency patients, 20 chronic disease anemia patients and 25 thalassemia minor patients whose hemoglobin levels were more than 9.5 gr/dl were included in the study. A positive correlation was found in the iron deficiency group between blood hemoglobin levels and MCV as well as MCH, while this positive correlation was not seen in patients with thalassemia minor. Erythrocyte counts of thalassemia minor group were higher than those of other two groups while MCV and MCH of this group were lower than those of others. The only difference between iron deficiency anemia and chronic disease anemia was that MCV and MCH values were lower in iron deficiency. Although RDW values of the three groups were higher than normal, there was no statistically significant difference among them.
