Çocukluk çağında nadir görülen bir pulmoner kist hidatik olgusu

Akciğer kist hidatiği Türkiye'de sık gözlenen paraziter bir hastalıktır. Ateş, prodüktif öksürük, göğüs ağrısı ve dispne şikayetleri ile başvuran altı yaşındaki kız hastanın solunum sistemi muayenesinde sağda ronkus ve solunum seslerinde azalma saptandı. Hastanın radyolojik incelemesinde sağ akciğerin alt kısmında iki adet örtüşen kistik lezyon ve sağda plevral effüzyon gözlendi. Ultrason ve bilgisayarlı tomografi sonuçları rüptüre pnömatosel veya akciğer apsesi olarak yorumlandı. Ultrason eşliğinde tanısal amaçla serbest plevral sıvıdan Örnek alındı. Torasentez materyali berrak sıvı (kaya suyu) görünümündeydi. Direk mikroskopik incelemede ise skoleksler gözlendi. Tekrarlanan akciğer grafisinde kist hidatiğin tipik bulgularından biri olan menisküs belirtisi saptandı. Operasyonda birisi plevraya birisi bronşa açılmış iki adet kist hidatik lezyonu mevcuttu. Her ikisine de parsiyel kistektomi ve kapitonaj uygulandı. Hastanın on sekiz aylık takibi sorunsuzdu. Akciğer kist hidatiği ayırıcı tanısı özellikle ülkemiz gibi endemik bölgede yer alan ülkelerde önem taşımaktadır. Olgu plevral mayiye yapılan torasentezin ve tekrarlanan akciğer grafilerinin önemini vurgulamak üzere bildirildi.

A rare case of pulmonary hydatid cyst in childhood

Pulmonary hydatid cyst is a disease which is frequently encountered in Turkey. A six year old girl was admitted with the complaints of fever, productive cough, chest pain and dyspnea. Physical examination revealed diminished respiratory sounds over the right lung and scattered ronchi. Chest X-rays showed two superimposed cystic lesions on the lower part of the right lung and pleural effusion. Ultrasonography and computerized tomography findings were interpreted as either ruptured pneumatoceles or lung abscesses. Diagnostic thoracentesis of the pleural effusion showed clear fluid. Scolecs were detected on direct microscopical examination. A repeated chest X-ray showed meniscus sign. During operation two hydatid cysts were observed one with pleural rupture and one with bronchial rupture. Both underwent partial cystectomy and capitonnage. The patient stayed symptom free during 18 months follow up. Pulmonary hydatid disease should be considered in the differential diagnosis of the patients presenting from endemic areas. This case was presented to emphasize the value of repeated plain roentgenograms and thoracentesis.
