Akut torakolomber kırıklarda transpediküler vida ile fiksasyon

Torakolomber kırıklarda hastanın ameliyat öncesi ayrıntılı incelenmesi, seçilecek yöntem ve tedavideki başarının temelini oluşturur. Manyetik rezonans görüntüleme (MRG) omurilik ve ligamentöz yapıların, bilgisayarlı tomografi (BT) ise kemik patolojinin ayrıntılı görüntülenmesine olanak sağlar. Bu çalışmada torakolomber kırık nedeniyle akut dönemde ameliyat edilen 20 olgu radyolojik bulgular ve cerrahi sonuçlar açısından incelendi. Ameliyat öncesi omurga yükseklik kaybı (% 60.7±% 12.4) ile ameliyat sonrası erken dönem (% 12.2±8.8) ve takipte saptanan yükseklik kaybı (% 15.7±%14.8) arasında anlamlı (p < 0.01) düzelme saptandı. Ameliyat öncesi ortalama kifoz açısı (24.8±13.0 derece) ile ameliyat sonrası erken dönem (6.4±3.9 derece) ve takip radyografileri (8.6±5.6 derece) arasında belirgin düzelme gözlendi. Ameliyat öncesi (% 52.6±21.3 ) ve ameliyat sonrası kanal içi bası (% 13.2±% 15.9) arasında anlamlı (p < 0.01) fark görüldü. Başvuruda yapılan nörolojik bakısında Frankel skoru (2.7±1.4) ile ortalama 15±8 aylık takipteki Frankel skoru (4.13±1.0 ) arasında belirgin fark bulundu (p < 0.01). Travma sonrası ortalama 49. saatte yaptığımız operasyon sonrası kanal içi basıda % 40 azalma saptandı. Akut torakolomber bölge kırıklarında transpediküler vida-çubuk sistemi uygulaması kolay, yeterli dekompresyona olanak sağlayan, skopi eşliğinde ve uygun teknik ile yapıldığında komplikasyon oranındaki düşüklük nedeni ile önerilen bir yöntemdir.

Transpedicular screw fixation in acute thoracolumbar fractures

Preoperative evaluation of patients with thoracolumbar fractures is the basis of the modality choice and success of the treatment. MPJ (Magnetic resonance imaging) reveals the spinal cord and ligamentous structures and CT (computed tomography) is useful in showing of the bone pathology. In this study, 20 cases with thoracolumbar fractures, operated at acute stage, have been examined on the point of view of radiological findings and surgical outcomes. There was a significant improvement (p<0.01) between the difference of preoperative vertebral height loss (60.7 %±12.4) and postoperative early stage (12.2 %± 8.8) and on the follow-up (15.7 %±14.8). A significant improvement (p < 0.01) was observed between preoperative mean kyphosis angle (24.8±13.0), postoperative early stage (6.4±3.9) and on the follow-up (8.6±5.6) radiographs. There was a significant improvement between the compression of spinal canal preoperatively (52.6 %±21.3) and postoperatively (13.2 %±15.9). There was a significant difference (p < 0.01) between Frankel scores at the onset (2.7±1.4) and at the follow-up (4.13±1.0) in approximately 15±8 months. A reduction of 40 % in spinal canal compression was determined after the operation that was performed at 49 th hours after the trauma. Transpedicular screw and rod system is a method of choice for acute thoracolumbar fractures because of practice of usage, allowance to sufficient decompression and low rate of complication when it was performed with a favorable technique and fluoroscopy guidance.
