Solunum yoğun bakım ünitemizdeki enfeksiyon etkenleri ve direnç sorunu
Amaç: Kliniğimiz solunum yoğun bakım ünitesi(YBÜ)'nde enfeksiyon nedeni olan bakterilerin dağılımı ve antibiyotik direnç oranlarını belirlemek. Yöntem ve Gereçler: 2005- 2007 yılları arasında YBÜ'mizde takip edilmiş hastaların dosya verileri retrospektif olarak incelendi. Bulgular: 146’sı KOAH zemininde olmak üzere toplam 257 hastadan 107’si tip I solunum yetmezliği, 94’ü tip II solunum yetmezliği, 30’u solunum arresti, 6’sı kardiyak arrest, 4’ü akut koroner sendrom, 3’ü akut serebrovasküler olay(SVO), 2’si ölümcül aritmi atağı, 1’i gastrointestinal sistemden(GİS) kanama nedeni ile YBÜ'ne alınmıştı. Toplam 257 kültür örneği değerlendirildi. Trakeal aspirat kültürlerininin %41'nde, balgam kültürlerinin %36'sında, idrar kültürlerinin %27'sinde, bronş lavajı kültürlerinin % 26'sında ve kan kültürlerinin % 22'sinde en az bir patojen izole edildi. Anlamlı üreme saptanan 80 hastanın 55’inde(%68.7) gram negatif, 25’inde(%31.2) gram pozitif bakteri kaynaklı enfeksiyon saptandı. En sık izole edilen etkenler; gram negatif grupta P. Aeruginosa (%25) ve A. Baumannii (%13.7) iken gram pozitif grupta MRSA( %20) idi. Hastaların 102’si toplum kökenli(TKP), 30’u hastane kökenli pnömoni(HKP) tanısı ile antibiyotik kullanmaktaydı. Klinik yanıt alınamayan 41 hastada 2. sıra, 10 hastada da 3. sıra antibiyotiğe geçiş gerekti. Balgamda üreyen P. Aeruginosa'nın %70 meropenem direnci olduğu saptandı. Pnömokok suşlarının sefuroksim, siprofloksasin ve imipenem dirençlerinin 2 yılda giderek artmakta olduğu görüldü. MRSA üreyen hastalarda halen en etkili antibiyotiğin vankomisin olduğu saptandı. P. Aeruginosa tedavisinde sık kullanılan piperasillin tazobaktam ve siprofloksasinin etkinliğinin devam ettiği görüldü. A. baumannii enfeksiyonlarında en etkili antibiyotiğin imipenem olduğu, piperasillin tazobaktam, vankomisin, amikasin ve siprofloksasin duyarlılığında geçmiş yıllara göre azalma olduğu saptandı. Klaritromisin, teikoplanin ve kolistin direncine rastlanmadı. Antibiyotik değişikliği yapılan 51 hastanın yoğun bakım ünitesinde kalış süresinin uzadığı saptandı( p 0.01). Sonuç: YBÜ'lerinde antibiyotiklere dirençli etkenlerin neden olduğu enfeksiyonlar yalnızca morbidite ve mortalite artışı değil aynı zamanda hastanede yatış süresinde uzama ve maliyet artışına neden olmaktadır. Direnç gelişimini önlemek için tedavide kullanılan antibiyotikleri akılcı seçmek gerekir
Infectious agents and the problem of resistance in respiratory intensive care unit
Aim: To determine the distribution of infectious pathogens and antibiotics resistance in our respiratory intensive care unit(ICU). Material And Methods: Data of patients, who were admitted to our ICU between 2005-2007, were evaluated retro- spectively. Findings: 257 patients, with 146 COPD, were admitted to our ICU by reason of; 107 type I respiratory failure, 94 type II respiratory failure, 30 respiratory arrest, 6 cardiac arrest, 4 acute coronary syndrome, 3 acute cerebrovascular accident (CVA), 2 episodes of fatal arrhythmia, 1 gastrointestinal tract bleeding. In total, 257 culture samples were evaluated. At least one pathogen was isolated from 41% of tracheal aspirate, 36% of sputum samples, 27% urine samples, 26% of bronchial lavage and 22% of blood cultures. 80 patients had positive cultures of which 55(68.7%) were gram-negative and 25(31.2%) were gram-positive bacterial infections. The most frequent isolated pathogens in gram-negative group were P. Aeruginosa (25%) and A. Baumannii (13.7%) whereas MRSA (20%) in gram-positive group. 102 patients with the diagnosis of community-acquired(CAP )and 30 hospital-acquired pneumonia (HAP) underwent any antibiotherapy. Second line antibiotherapy was administered to 41 patients and 3rd line to 10 patients who were clinically unresponsive to ongoing therapy. Antibiotics susceptibility tests for P. Aeruginosa showed 70% resistance to meropenem. Cefuroxime, ciprofloxacin and imipenem resistance of Pneumococcal strains was found to be increasing for past 2 years. Vancomycin was still the most effective antibiotics for patients with MRSA. For treatment of P. Aeruginosa, frequently used piperacillin tazobactam and ciprofloxacin has still conserved their effec- tiveness. Imipenem was the most effective antibiotic for A. baumannii infections whereas sensitivity for piperacillin tazobactam, vancomycin, amikacin and ciprofloxacin was found decreasing when compared with previous years. Resistance to claritromisin, teicoplanin and colistin was not determined. Antibiotics substitution was resulted with extended hospitalisation in the ICU ( p=0.013). Result: Antibiotics resistant infections in the ICU are related with not only increased mortality and morbidity but also extended hospitalisation and high costs. Choosing the correct antibiotherapy is a necessity to provide decrease in bacterial resistance. siv
