Diyabet hastalarının ruhsal sorunlarının Kaynakların Korunması kuramı kapsamında incelenmesi

ÖZET Amaç: Diyabet, her geçen gün yaygınlığı artan önemli bir hastalıktır. Diyabet hastaları fiziksel sorunlar yanında pek çok ruhsal sorunla karşılaşmakta ve bu sorunlar hastaların tedavi süreçlerini de olumsuz etkileyebilmektedir. Hastaların yaşadığı ruhsal sorunların düzeyi ve ilişkili olduğu faktörler belirlenerek tedavi edilirse fiziksel tedavileri de daha sağlıklı yürütülebilir. Bu nedenle, bu araştırmanın amacı Tip 2 diyabet hastalarının kaygı, depresyon ve genel ruhsal sorunlarıyla (kaygı+depresyon toplam puanı) ilişkili faktörlerin (baş etme yolları, algılanan sosyal destek, öz yeterlik, duygu dışa vurumu ve kaynak kaybı) Kaynakların Korunması kuramı kapsamında incelenmesidir. Hastalar ve Yöntem: Araştırmaya toplam 116 diyabet hastası katılmıştır. Sosyo-demografik ve Hastalıkla İlgili Bilgi Formu’nun yanı sıra 6 ayrı ölçek uygulanmıştır. Değişkenler arası ilişkilerin incelenmesi için bağımsız gruplar için t testi, korelasyon ve regresyon analizleri yapılmıştır. Bulgular: Diyabet hastalarının yaklaşık yarısının yüksek düzeyde kaygı ve depresyon yaşadıkları bulunmuştur. Çaresiz baş etme ve kaynak kaybının ruhsal sorunların tümüyle pozitif yönde ilişkili olduğu, iyimser baş etmenin ise tüm ruhsal sorunlarla negatif yönde ilişkili olduğu bulunmuştur. Algılanan duygu dışavurumunun, duygusal aşırı ilgilenme boyutundaki artış ise sadece genel ruhsal sorunlardaki azalmayla ilişkili bulunmuştur. Sonuç: Psikolojik müdahale ve destek programlarında araştırmada belirlenen konulara odaklanılması yararlı olacaktır. Anahtar Kelimeler: Tip 2 diyabet, Depresyon, Kaygı

Objectives: Diabetes is an important disease with an every day increasing prelevance. Diabetes patients experience psychological problems as well as physical problems which may negatively influence treatment process. If the psychological distress level of patients and related factors were determined, more healthy physical treatments could be carried out. For this reason, the aim of this study was to assess anxiety, depression and psychological distress (total score of anxiety+depression) and the related factors (i.e., ways of coping, perceived social support, self efficiency in relation to diabetes, expressed emotions, and loss of resources) among patients with Type 2 diabetes in the light of the scope of Conservation of Resources theory.Patients and Methods: Sociodemographic and Illness Information form and also six different scales about psychological distress and related factors were administered to 116 diabetes patients. To investigate the relationship between the variables the independent samples t test, regression and correlation analysis were performed.Results: It was found that nearly half of the diabetes patients exprienced high levels of anxiety and depression. More importantly, it was observed that helplessness coping and resource loss were positively related to all psychological problems. However, optimistic coping was negatively associated with such psychological problems. In addition, it was indicated that increased emotional over-involvement domain of expressed emotion was only associated with decrease in general psychological distress.Conclusion: It was found that resource loss, coping strategies and expressed emotions were found to be related to psychological distress. It was suggested that psychological intervention programs should focus on issues identified in this study
