Psikiyatri Kliniğinde Yatan Hasta Bireylerin Yaşamı Sürdürme Nedenleri ile İntihar Davranışı Arasındaki İlişki: Sistematik Derleme

İntihar ile ilgili yapılan çalışmalar genellikle, kişilerin “olumsuz” bilişleri üzerinde odaklanırken, Linehan ve ark.’nın geliştirdiği Yaşamı Sürdürme Nedenleri Envanteri, kişileri intihar davranışına karşı koruyan “olumlu” bilişler üzerinde odaklanmaktadır. Bu sistematik derleme, psikiyatri kliniğinde yatan hasta bireylerin yaşamı sürdürme nedenleri ile intihar davranışı arasındaki ilişkiyi ve yaşamı sürdürme nedenlerinin intihar davranışına karşı koruyucu olup olmadığını belirlemek için yapılmıştır. Bu derleme elektronik tarama temel veritabanları olan Pubmed, Science Direct ve PsycInfo aracılığı ile Ocak 2016-Ağustos 2017 yılları arasında yürütülmüştür. Araştırmalardan çalışmanın amacına uygun olanlar belirlenmiştir (n=14). İncelenen bu makalelerde psikiyatri kliniğinde yatarak tedavi gören hasta bireylerin intihar davranışlarına yönelik değerlendirmeler yapılarak intihar düşüncesi ve/veya intihar girişimini etkileyen faktörler araştırılmıştır. Dürtüsellik, saldırganlık, umutsuzluk, depresyon, madde veya alkol kullanım öyküsü, çocukluk ve ergenlik döneminde kötüye kullanım öyküsü, ruhsal acı, ailede intihar davranışının bulunması, intihar girişimi öyküsü, borderline kişilik bozukluğu öyküsünün intihar davranışını artıran faktörler olduğu saptanmıştır. Yaşamı sürdürme nedenlerinin intihar davranışına yönelik koruyucu bir faktör olduğu ve yaşamı sürdürme nedenleri arttıkça intihar olasılığının azalabileceği ileri sürülmüştür

The Investigation of The Relationship Between Reasons for Living and Suicidal Behavior in Psychiatric Inpatients: A Systematic Review

While suicide-related studies generally focus on the “negative” cognition of people, the Reason for Living Inventory, developed by Linehan et al., focuses on the “positive” cognition that protects people from suicidal behavior. This systematic review was carried out to determine whether the relationship between suicidal behaviors and the reasons for living of individuals living in psychiatric clinics is protective against suicidal behavior. This review was conducted between January 2016 and August 2017 through the electronic databases Pubmed, Science Direct, and PsycInfo and includes 14 studies. Factors affecting suicidal thoughts and/or suicide attempts were investigated by evaluating the suicidal behaviors of the patients who were treated in the psychiatric clinic. It has been determined that the narratives of impulsivity, aggression, hopelessness, depression, substance or alcohol use, childhood and adolescent abuse history, mental pain, presence of family suicide behaviors, suicide attempt narratives, and borderline personality disorders are factors that increase suicidal behaviors. Reasons for living are protective factors for suicidal behaviors. It is suggested that as the reasons for living increase, the possibility of suicide may decrease.
