Epilepsili Kadınlarda Gebelik ve Hemşirelik Bakımı
Epilepsi hastalığı olan bireylerdeki gebelikler, yüksek riskli gebelikler içerisinde değerlendirilmektedir. Bu yüzden epilepsi hastaları için en ideal olanı planlı bir gebeliktir. Epilepsi hastalarında, nörolog ve kadın-doğum uzmanının birlikte ortak bir karara varması gerekmektedir ve epilepsili kadınlar gebelik komplikasyonları ile daha sık karşılaşabileceği konusunda bilgilendirilmelidir. Konjenital malformasyon açısından, monoterapi alan gebeler politerapi alan gebelere göre daha az risklidir. Son yıllarda yapılan birçok çalışmada topiramat, valproat ve fenobarbital gebelik süresince riskli ilaçlar olarak; okskarbazepin, levetirasetam ve lamotrijin ise az riskli ilaçlar olarak değerlendirilmiştir. Konjenital malformasyon ve fetal anomali yönünden epileptik anne adaylarının tarama ve tanı testleri ve rutin kontrolleri yapılmalıdır. Ayrıca epileptik anne adayları antenatal bakımını yaptırmalıdır. Bebekler doğunca IM K vitamini yapılmalıdır. Epileptik anneler ve yeni doğan bebekler için emzirme daha büyük bir önem taşımaktadır. Bu yüzden doğum yapan epilepsili anneler, emzirme danışmanlığı ile cesaretlendirilmeli ve desteklenmelidir. Doğum yapan anneler doğumdan 3-4 ay sonra da nörolog ile görüşmelidir. Abstract Pregnancies of women with epilepsy are regarded high risk pregnancies. Therefore, a planned pregnancy is the most ideal way for epilepsy patients. A neurologist and an obstetrician should reach a joint decision about an epilepsy patient, and epilepsy patients should be informed that they may encounter complications more frequently. With respect to congenital malformations, pregnant women who receive monotherapy are less risky than those receiving polytherapy. Recently, topiramate, valproate and phenobarbital have been considered as risky drugs during pregnancy, while oxcarbazepine, levetiracetam and lamotrigine are considered low risk drugs. Screening and diagnostic tests, and routine controls of epileptic mothers should be done to detect congenital malformation and fetal anomaly. Additionally, epileptic mothers should have antenatal care. When babies are born, IM vitamin K should be administered. For epileptic mothers and newborns, breastfeeding is of greater importance. Therefore, mothers with epilepsy who give birth should be encouraged and supported by breastfeeding counseling. Mothers who give birth should see a neurologist 3-4 months after birth.
