DİYALİZE GİRMEYEN KRONİK BÖBREK HASTALIĞINDA HEPATİT B AŞISI SONRASI ANTİKOR YANITINA ETKİ EDEN FAKTÖRLERİN DEĞERLENDİRİLMESİ

Giriş: Bu çalışmanın amacı diyalize girmeyen kronik böbrek hastalarında dört doz Hepatit B aşısı sonrası antikor yanıtına etkili demografik ve klinik faktörlerin değerlendirilmesidir. Gereç ve Yöntem: Bu çalışma tek merkezli, retrospektif ve longutidinal bir çalışma olarak tasarlandı. Çalışmaya KBH evre 1-5 olan toplam 251 hasta dahil edildi. Hastalara dört doz (başlangıç, 1., 2. ve 6.ay) ve 40 µg hepatit B aşısı her iki deltoid kasa eşit dozda intramüsküler olarak uygulandı. Aşı sonrası üç ay ve ortalama 44,5 ay sonra AntiHBs antikor titresi değerlendirildi. AntiHBs antikor titresi≥10 IU/L olanlar yanıtlı,0.05).. Sonuç: Hepatit B aşısı sonrası antikor yanıtına glomeruler filtrasyon hızı ve KBH evresinin etkisi yoktur. Malnutrisyon aşıya yanıtı etkileyen önemli bir parametredir. KBH hastaları erken evrelerden itibaren aşılanmalıdır.

EVALUATION OF THE FACTORS AFFECTING ANTIBODY RESPONSE AFTER HEPATITIS B VACCINE IN NON-DIALYSIS CHRONIC KIDNEY DISEASE

Introduction: The aim of this study is to evaluate the demographic and clinical factors affecting the antibody response after four doses of Hepatitis B vaccine in patients with chronic kidney disease not undergoing dialysis. Material and Method: This study was designed as a single center, retrospective and longutidinal study. A total of 251 patients with CKD stage 1-5 were included in the study. Four doses (initial, 1st, 2nd and 6th month) and 40 µg hepatitis B vaccine were administered intramuscularly in equal doses to both deltoid muscles. AntiHBs antibody titer was evaluated three months after vaccination and 44.5months on average.Those with antiHBs antibody titres ≥ 10 IU/L were considered responsive, those 0.05). Conclusion:Glomerular filtration rate and CKD stage have no effect on antibody response after hepatitis B vaccination. Malnutrition is an important parameter that affects the response to the vaccine. CKD patients should be vaccinated from the early stages.
