Proseal laringeal maske kullanılan bir çocukta gelişen laringeal ödem

Amaç: Larenks ödemi, vokal kordlar, vokal bandlar, aritenoidler, epiglot ve subglottik bölgede meydana gelen enflamasyon sonucu mukozadaki şişmedir. Hava pasajında meydana gelen darlık nedeniyle hayatı tehdit edecek derecede akut bir patoloji olabildiği gibi yıllarca düzelmeyen kronik bir hastalık olarak da görülebilir. Bu olguda proseal laringeal maske (PLMA)’nın altta yatan bir nedene bağlı olarak laringeal ödemi tetikleyebileceğini sunmayı amaçladık. Olgu sunumu: ASA I risk grubuna giren 10 aylık süt çocuğuna sünnet revizyonu amacıyla genel ve kaudal anestezi planlandı. Fizik muayene, laboratuar ve PA akciğer grafisi normaldi. Herhangi bir problem yaşanmadan cerrahi prosedür bitirildi ve PLMA ekstübasyonu yapıldı. Aşırı sekresyon, paradoksik solunum ve stridor gelişmesi üzerine yapılan laringoskopi esnasında larinksde aşırı ödemden dolayı glottik açıklığın tamamen kapandığını görüldü. Bunun üzerine prednol ve süksinilkolin enjeksiyonunu takiben reentübasyon yapıldı. Larinks ödeminden dolayı i.v furosemid 5 mg ve 36 mg teofilin infüze edildi. Yaklaşık 4 saat sonra ekstübe edilen hasta yoğun bakıma alındı. Yoğun bakımda 48 saat takip edildikten sonra taburcu edildi. Sonuç: PLMA nadir de olsa hayatı tehdit eden komplikasyonlara neden olan olayları tetikleyebilir.

Laryngeal edema secondary to use of a proseal laryngeal mask airway in a child

Objective: Laryngeal edema can occur due to swelling of vocal cords, vocal bands, arytenoids, epiglottal and subglottic area. It can be a life threatening condition due to air passage blockade, and may be seen as longstanding chronic disorder. Case report: General and caudal anesthesia was planned in an ASA I risk group 10-month-old child for revision of circumcision. Physical examination, laboratory work up and chest X-ray revealed no abnormality. Surgical procedure was finished without any problem and the PLMA was removed. Excessive secretion, paradoxical breathing and stridor on the development of edema due to excessive glottic space the larynx during laryngoscopy is closed completely seen. Following the injection of succinylcholine and prednol reentübasyon was performed. Furosemid (5 mg) was given and theophylline (36 mg) was intravenously infused for the management of laryngeal edema. Conclusion: Rarely life-threatening PLMA cause complications that can trigger events.
