Kronik obstrüktif akciğer hastalarında sigara içme, akciğer fonksiyonları ve dispne algılaması arasındaki ilişki

Amaç: Sigara içimi kronik obstrüktif akciğer hastalığının (KOAH) gelişiminde eri önemli faktördür. Bu çalışmada, KOAH'lı hastalarda, sigara içiminin havayolu obstrüksiyonuna etkileri ve sigara içimi ile dispne algılaması arasındaki ilişki araştırıldı. Yöntem: Çalışmaya KOAH tanısı ile izlenen 70 erkek olgu katıldı. Olguların sigara içme öyküsü kaydedildi. Solunum fonksiyon testi yapıldı. Dispne algılaması modifıye Medical Research Council (MRC) dispne skalası ile değerlendirildi. Bulgular: Birinci saniyedeki zorlu ekspirasyon volümü (FEVı), tepe akım hızı (PEF), zorlu ekspirasyon akımının % 25-75 (FEF%25-7s) değerleri ile sigara öyküsü arasında istatistiksel olarak anlamlı bir ilişki bulundu. Sigara içimi ve MRC dispne skalası arasında istatistiksel olarak anlamlı bir ilişki gösterilemedi. Sonuç: KOAH'lı hastalarda sigara içimi ve MRC dispne skalası ile ölçülen dispne algılaması arasında ilişki saptanmadı. Bununla beraber, sigara içimi ile büyük ve orta havayollarında obstrüksiyon oluştuğu tespit edildi. Bu olgularda sigaranın bırakılması önemlidir.

Relationship between smoking, lung functions and dyspnea perception in patients with chronic obstructive pulmonary disease

Objective: Smoking is the most important factor in the development of chronic obstructive pulmonary disease (COPD). The purpose of this study was to investigate the effects of cigarette smoking on airflow obstruction, and the relationship between smoking and dyspnea perception in patients with COPD. Methods: Seventy patients with clinically stable COPD participated in this study. Smoking history was recorded. Pulmonary function test was performed. Dyspnea perception was measured by modified Medical Research Council (MRC) dyspnea scale. Results: There was a significant relationship between smoking and forced expiratory volume in one second (FEVı), peak flow rate (PEF) and forced expiratory volume from 25% to 75% (FEF%25-7s). Smoking was not significantly related with MRC dyspnea scale. Conclusion: .There was no significant relationship between smoking and dyspnea perception by MRC dyspnea scale in patients with COPD ln addition, smoking results in large and middle airway obstruction. Smoking cessation is important in these patients.
