Arteria carotis internanın pars cavernosasının dalları: Mikroanatomik çalışma

Amaç: Sinüs cavernosus içerisinde arteria carotis internadan (ACİ) çıkan arterlerin dağılımı, sıklık oranı ve kraniyal sinirler ile ilişkileri, bu sinirlerin kanlanmasının tespit edilmesi ve bu bölgede yapılacak operatif yaklaşımlara katkı sağlamak amaçlanmıştır. Yöntem: Çalışma; 14 erişkin kadavrasında; ACİ'ya diseksiyon öncesi kırmızı lateks enjekte edilerek, olympus operasyon mikroskobu ve stereo mikroskopla gerçekleştirildi. ACİ'nin pars cavernosasmın dalları Nomina Anatomica esas alınarak tanımlandı. Bulgular: Truncus meningohypophysialis tüm kadavralarda bulundu, incelenen vakaların % 64.2'sinde truncus meninghypophysialisden ramus basalis tentorii, ramus meningealis ve a. hypophysialis inferior birlikte ayrılmaktadır. Ramus meningeus; % 35.7 medial dal, % 21.5 lateral dal ve % 42.8 medial ve lateral beraber olarak ayrılan tip tespit edildi. A. hypophysialis inferior, kadavraların % 89'unda bulundu. A. sinüs cavernosus inferior, % 78.5 ACİ'nin pars cavernosa'sının horizontal kısmından, % 7 truncus meningohypophysialisden orijin alırken, % 14.5 vakada tespit edilemedi. A. capsularis vakaların % 14.5'inde tespit edildi. Sonuç: ACİ'dan sinüs cavernısus içerisinde çıkan arterlerin dağılımı, sıklığı ve kraniyal sinirler ile olan ilişkileri bölge ile ilgili cerrahi operasyonlarda kullanılacak yararlı bilgiler sağlayabilir.

The branches of the cavernous part of the internal carotid artery, microanatomical study

Objective: To examine the branches of arteries originated from the internal carotid artery (ICA) in the cavernous sinüs, their distribution, frequency ratio, relationships with the cranial nerves and the blood supply of the related nerves to give more anatomical data for surgical approach. Methods: 14 adult human cadavers were previously injected with red latex before dissection under the olympus operation and stereo microscope. Results: The meningohypophysial trunk was found in all cadavers. in 64.2% of cases, the tentorial basal and meningeal branches together with inferior hypophysial artery were originated from the meningohypophysial trunk. The meningeal branch was observed, as medial branch in 35.7%, lateral branch in 21,5% and as two separate branches in 42.8%. The inferior hypophysial artery was seen in 89% of the cadavers. The inferior cavernous artery was observed in 78.5% originated from the horizontal division of the cavernous part of the ICA, in 7% from meningohypophysial trunk and it was not observed in 14.5%. The capsular artery was also determined in 14.5% of the cadavers. Conclusion: The distribution and the frequency of the arteries originated from the ICA in the cavernous sinus and their relationship with the cranial nerves may provide useful background Information for the surgical operations related with this region.
