Trakea rekonstrüksiyonu: Dakron ve PTFE greftlerinin karşılaştırılması (deneysel çalışma)

Amaç: Tavşanlarda uzun trakeal segment rezeksiyonunu takiben rekonstrüksiyon yöntemlerinden primer sütür ile dakron ve PTFE greftlerini birbirleriyle kıyaslamaktır. Yöntem: Her birinde 10 tavşan bulunan 3 grup oluşturuldu. Grup A kontrol grubu idi ve bu gruba 8 halka içeren trakea rezeksiyonunu takiben primer anastomoz yapıldı. Grup B'de birinci gruptaki gibi eşit uzunlukta trakea rezeksiyonunu takiben dakron greft ile anastomoz uygulandı. Grup C'de ise grup B'de yapılan işlemler uygulandı ve greft olarak PTFE kullanıldı. Bulgular: Grup A'da hiç stenoz görülmedi. Hafif derecede stenoz gösteren grup B grup C'ye kıyasla daha az stenotik bulundu. Epitelizasyon ve neovaskülarizasyon ise grup A'nın ardından grup B'de grup C'ye kıyasla daha iyi bulundu. Sonuç: Trakea rekonstrüksiyonlarında primer anastomoz uygulama olanağı varsa öncelikle bu yöntem denenmelidir. Ancak primer anastomoza olanak vermeyen daha uzun segment rezeksiyonlarında protez kullanılacaksa dakron tercih edilebilir.

Tracheal reconstruction: Comparison of dacron and PTFE grafts (Experimental study)

Object: To compare the efficacies of tracheal reconstruction methods consisting primary end to end anostomosis, dacron and PTFE graft transpositions following long segmental tracheal resection in rabbits. Methods: Three different groups, each consisting 10 rabbits were developed. Group A was the control group and primary anastomosis was performed after resection of 8 tracheal rings. Group B: Dacron graft transposition was performed in this group after the same tracheal resection as in the first group. Group C: PTFE graft transposition was used in this group for the same tracheal resection as in group B. Results: No tracheal stenosis was seen in the first group. in the second group, mild stenosis was found which was considerably less stenotic than the third group. Epithelization and neovascularization were best in the dacron used group in comparison of the prostheses. Conclusion: If possible primary anostomosis should be preferred in the tracheal reconstructions, however, when primary anostomosis is impossible and needed cases long segment tracheal resections are encountered dacron graft should be preferred for reconstruction.
