Insulin resistance and lipid profile in polycystic ovary syndrome

Amaç: Son yıllarda insulin rezistansının polikistik over sendromunu başlatan ve/veya eşlik eden bir faktör olduğu önerilmektedir. Polikistik over sendromlu hastalarda ateroskleroz muhtemelen eşlik eden insulin rezistansına bağlı olarak sık görülür. Bu çalışmanın amacı polikistik över sendromu olan olgularda insulin rezistansı derecesini ve lipid profilini incelemektir. Metod: Çalışmaya polikistik över sendromu tanısı alan 64 hasta (ortalama yas: 23.04±5.5) ve 21 sağlıklı kontrol (ortalama yaş: 27.31±8.3) alındı. Tüm olguların fizik muayene, biyokimyasal ve hormon tetkikleri ile abdomen ve pelvik ultrasonografileri yapıldı. Periferik insulin rezistansı öglisemik-hiperinsülinemik klemp testi ile araştırıldı.Sonuçlar: Polikistik över sendromu olan olguların bel-kalça oranı, LH, serbest testosteron, androstenedion düzeyleri, LH/FSH oranı kontrol grubuna göre yüksek; seks hormon bağlayıcı globulin ve FSH düzeyleri ise kontrol grubuna göre düşük bulundu. Polikistik över sendromu olan olguların %19.3'ünde ciddi insulin rezistansı mevcuttu. Insulin rezistansı arttıkça LDL-kolesterol düzeylerinde artış ve HDL-kolesterol düzeylerinde azalma saptandı. Yorum: Polikistik över sendromunda insulin rezistansı varlığında, yüksek LDL-kolesterol ve düşük HDL-kolesterol düzeyleri bu hastalarda görülen erken aterosklerozda rol oynayabilir.

Polikistik over sendromunda insülin rezistansı ve lipid profili

Purpose: In recent years, it has been suggested that insulin resistance (IR) might be an initializing and for contributing factor for the polycystic ovary syndrome (PCOS). Many studies reported that arteriosclerosis is a coexisting pathology in PCOS probably due to existing IR. The aim of the study was to investigate the degree of IR and lipid profile in patients with PCOS. Methods: Sixty-four patients with PCOS (mean age: 23.04±5.5) and 21 healthy controls (mean age: 27.31 ±8.3) were included in the study. Physical examination, routine biochemical analysis, ovary and adrenal gland hormone assays and abdomen and pelvic ultrasonography were performed in all cases. Peripheral IR was investigated by euglycemic-hyperinsulinemic clamp test (EHCT) in both groups.
