Çekim Boşluğunun Korunmasında Trombositten Zengin Fibrinin Klinik Ve Histolojik Etkinliğinin Değerlendirilmesi: Olgu Sunumu

Amaç: Diş çekimi sonrası iyileşme döneminde alveol kemikte rezorpsiyon oluşur. Trombosit kaynaklı büyüme faktörlerinin yara iyileşmesi üzerindeki potansiyel etkisinden yola çıkarak, günümüzde trombositten zengin fibrinin (TZF) çekim boşluğunu koruma amaçlı kullanımı gündeme gelmiştir. Bu olgu raporunda, diş çekimi sonrası, alveol sokete uygulanan TZF'nin klinik ve histolojik sonuçlarının değerlendirilmesi amaçlanmıştır. Yöntem: 45 yaşındaki kadın hasta sol alt birinci molar dişinde restore edilemeyen kron-kök kırığı nedeniyle kliniğimize başvurdu. Dişin atravmatik çekiminin ardından soket TZF ile dolduruldu. Alveol kemik yüksekliği ve genişliği klinik olarak başlangıçta ve 3. ayda kaydedildi. Diş çekiminden 3 ay sonra implant cerrahisi sırasında sert doku biyopsisi alındı. Bulgular: Diş çekimi sonrası herhangi bir komplikasyon ile karşılaşılmadı. İmplantın yerleştirilmesi sırasında alveol kemiğin boyutlarının korunduğu gözlendi. Histolojik incelemede, kemikte yeni lameller yapı ile birlikte değişken miktarlarda damarlanma gözlendi. Kemik lakünleri arasında çok sayıda osteosit mevcuttu. Sonuç: Bu olgu raporu, TZF uygulamasının çekim boşluğunun korunmasında etkili bir biyomateryal olarak kullanılabileceğini klinik ve histolojik olarak ortaya koymuştur. Çekim boşluğunun korunmasında TZF uygulamasının etkinliğinin belirlenmesi için randomize klinik kontrollü çalışmalara ihtiyaç vardır.

Clinical And Histological Evaluation Of Post-Extraction Platelet Rich Fibrin Socket Filling: A Case Report

Objectives: Alveolar bone resorption generally occurs during healing after tooth extraction. The healing potential of platelet growth factors has generated interest in using platelet-rich fibrin (PRF) in socket preservation procedures. The aim of this case report is to investigate whether the use of PRF for socket filling could improve bone tissue quality of the alveolar bone following tooth extraction. Methods: A 45 year-old-female patient presented to our clinic with a non-restorable mandibular molar due to subgingival oblique crown-root fracture. After extraction of a tooth using a flapless atraumatic technique, socket was filled with PRF. Implant placement was performed at month 3. A bone biopsy was obtained for histomorphometric analysis. Results: Clinical healing was uneventful. The outcome of the convex ridge allowed appropriate conventional implant placement. Formation of new lamellar structure was found in histologic evaluation. The specimen showed a new bone presented with variable amounts of loose, fibrous stroma filled with fibrovascular tissue. Conclusions: Clinically and histologically, this report demonstrated that PRF could be used as an effective biomaterial in extraction site preservation prior to implant placement. Randomized controlled clinical trials are needed to fully evaluate the usefulness of PRF in socket preservation after tooth extraction
