Sistemik fizik muayene sırasında kan basıncı ne zaman ölçülmeli?
Kan basıncı ölçümü fizik muayene işleminin önemli bir parçasıdır. Genellikle kan basıncı fizik muayenenin başlangıcında ölçülmektedir. Oysa yeterince uygun bir kan basıncı ölçümü yapabilmek için, fizik muayene işleminin hangi aşamasının daha uygun olduğu yeterince irdelenmemiştir. Bu nedenle yaptığımız çalışmamızda, değişik yakınmalarla polikliniğimize ilk kez gelen, semptomatik kardiyovasküler bir hastalığı olmayan ve kardiyovasküler etkili herhangi bir ilaç kullanmayan 252 hastada kan basınçları, fizik muayene başlangıcında ve sonunda JNC-VI (The Sixth Report of the Joint National Committee on Detection, Evaluation, and Treatment of High Blood Pressure) önerilerine uygun olarak ölçülmüştür. Erkek ve kadın hastaların yaş ortalaması sırasıyla, (Ort ±SD) 46.2±14 ve 43.7±12 olup anlamlı bir fark göstermemiştir (p= 0.234). Olgularda ilk ve ikinci sistolik kan basıncı ortalamaları 139.9±25 mmHg ve 135.1±25 mmHg (p=0.001), ilk ve ikinci diyastolik kan basıncı ortalamaları ise 87.2±13 mmHg ve 86.0±73 mmHg bulunmuştur (p= 0.041). Sistolik ve diyastolik kan basınçları fizik muayene sonundaki ölçümlerde anlamlı olarak daha düşük saptanmıştır (p= 0.001 ve p= 0.041). Sistolik kan basıncı farkı 30 - 60 yaş arası hastalarda daha anlamlı bulunmuştur (p= 0.001). Sonuç olarak kan basıncı ölçümü, her sistemik fizik muayenenin sonunda yapılırsa daha yararlı olabileceği kanısındayız.
When should blood pressure be measured during the systemic physical examination of a patient?
Blood pressure measurement is an important part of physical examination procedure. Generally blood pressure measurements are done at the beginning of the physical examination, however, the most suitable step of the systemic physical examination for obtaining an accurate blood pressure measurement have not been discussed enough. Therefore, we performed a population based study, on the 252 out patients who were administered to hospital for the first time with different complaints. Their blood pressure was measured according to the JNC-VI report at the beginning and at the end of the systemic physical examination. None of them had symptomatic cardiovascular disease or under any cardiovascular drug therapy. Mean T SD ages of male and female patients (46.2±14 and 43,7±12 years) were not significantly different (p>0.05). Means of first and second systolic blood pressure were 139.9±25 mmHg and 135.1±25 mmHg (p=0.001). Means of first and second diastolic blood pressure were 87.2±13 mmHg and 86.0±13 mmHg (p=0.041). Systolic and diastolic blood pressure were significantly lower at the end of the systemic physical examination (p<0.05). The systolic blood pressure differences were more significant in patients aged between 30 and 60 years (p<0.001). In conclusion, we thought that it should be better if blood pressure measurement were performed at the end of the each systemic physical examination.
