Posttravmatik progresif renal atrofi: Olgu sunumu

Geçirdiği trafik kazasının ardından Aydın Devlet Hastanesi'nde splenektomi uygulanan ve daha sonra İzmir Behçet Uz Çocuk Hastanesi'ne sevk edilen 1991 doğumlu erkek olguya burada yapılan tetkiklerinde sol post travmatik renal arter trombozundan şüphelenilmiş, olgu daha sonra gelişen komplikasyonlar nedeniyle ikinci kez opere edilmiştir. Takipte sol böbrekte atrofi geliştiği saptanmış, ancak bu konuda bilgilendirilmeyen aile ameliyat sırasında sol böbreğin izinsiz olarak çıkarıldığı iddiasıyla şikayette bulunmuştur. Olgunun durumunun incelenmesi ve ameliyat sırasında böbreğin çıkarılıp çıkarılmadığının tespiti amacıyla Ege Üniversitesi Tıp Fakültesi Adli Tıp Anabilim Dalından ilgili savcılık tarafından rapor istenmesi üzerine olgu muayene edilmiş ve tüm hastane evrakı incelenmiştir. Ameliyattan sonraki kontroller sırasında çekilen USG'lerde de böbreğin görülüyor olması nedeni ile sol renal arter yaralanmasına sekonder olarak gelişen nadir ve geç bir komplikasyon olan posttravmatik renal atrofi olduğuna karar verilmiştir.

Posttraumatic progressive renal atrophy: Case report

A male case born in 1991 that underwent splenectomy in Aydın State Hospital after a traffic accident is reported. He was transported to Dr. Behçet UZ Children's Hospital and posttraumatic thrombosis of left sided arteria renalis was suspected in this hospital and the patient was re-operated because of later complications. Left renal atrophy was observed during follow up. The family of the child was complainant claiming that his left kidney was taken during operation without permission. The court asked for knowledge from Ege University Forensic Medicine Department for decision about this case whether left kidney was taken during operation. The case was medically and radiologically examined and all the medical reports were evaluated. Ultrasonographic examination during postoperative follow up revealed the presence of left kidney. The diagnosis was posttraumatic renal atrophy as a late complication of left renal injury.
