Çocuklarda 2 yıllık Holter monitörizasyon deneyimi

Çocukluk çağında kardiyak ritm bozuklukları özellikle postoperatif olguların arttığı son yıllarda önemli bir sorun olarak karşımıza çıkmaktadır. İstirahatte çekilen elektrokardiyografi, kardiyak ritmin kısa bir dönemi için bilgi vermekte ve bir çok ritm bozukluğu gözden kaçabilmektedir. Bu retrospektif çalışma ile olguları incelenerek, 24 saat Holter Monitörizasyon endikasyonları gözden geçirilmiş, Pediyatrik Kardiyolojide tanı ve tedavideki yeri vurgulanmak istenmiştir. Ege Üniversitesi Tıp Fakültesi Pediyatrik Kardiyoloji Bilim Dalında Şubat 1999 ile Mart 2001 tarihleri arasında 367 hastaya yapılan toplam 490 adet 24 saatlik Holter Monitörizasyon kayıtları retrospektif olarak incelendi. Hastaların yaşları 6 gün ile 28 yaş arasında değişiyordu, ortalama yaş 11±5 yıl idi. Olguların Holter monitörizasyonları asemptomatik disritmik grup, semptomu olan disritmisi olmayan grup ve diğerleri (semptom ve disritminin birlikte olduğu grup ve pacemaker kontrolleri) olmak üzere 3 grupta toplandı. Değerlendirilen olguların %54'ü normal bulunurken, %46'sında değişik spektrumda disritmi saptandı. Bu bulguların dağılımı; ventriküler disritmiler %25, supraventriküler disritmiler %10, supraventriküler ve ventriküler disritmiler birlikte %5, komplet A-V blok %4, uzun QT sendromu %2 oranında idi. Semptomatik çocuklarda Holter Monitörizasyon esnasında genellikle belirti olmaması nedeniyle ritm bozukluklarını belirlemek güç olmaktadır. Ancak klinik bulgusu şüpheli ve altta yatan konjenital kalp hastalığı olan olgularda tedavi gerektirecek bulguların saptanmış olması nedeniyle, disritmi şüphesi olan durumlarda Holter monitörizasyonun önemli bir tanı aracı olduğunu düşünmekteyiz.

Two years experience of Holter monitoring in children

Cardiac arrhythmia in pédiatrie patients appears as a significant problem in recent years, with the increase in the number of post-operative cases. The resting electrocardiograph^ findings give information only about a short period of cardiac rhythm, and accordingly most of the arrhythmias may be skipped. In this retrospective study, we analysed the prognostic significance and effectiveness of 24-Hour Holter monitoring recordings in pédiatrie cardiac patients. In this study, 490 Holter monitoring recordings obtained from 367 cases in Ege University Medical Faculty Pédiatrie Cardiology Department, from February 1999 to March 2001 were analysed retrospectively. The age of the patients were between 6 days and 28 years and their average age was 11±5 years. The Holter monitoring FQcordings of the patients were divided into three groups as; asymptomatic dysrhythmic group, symptomatic non-dysrhythmic group and both symptomatic and dysrhythmic group. While 54% of the cases were normal, dysrhythmia of a wide spectrum was observed in 46% of the cases. The distribution of these findings were ventricular dysrhythmia 25%, supraventricular dysrhythmia 10%, supraventricular and ventricular dysrhythmia together 5%, complete A-V block 4%, and Long QT syndrome was 2%. In symptomatic cases, it was difficult to determine cardiac arrhythmia during Hotter Monitoring since no symptom was observed. Nevertheless, we consider that the prognostic significance of Hotter monitorization is high in the cases who are suspicious about dysrhythmia.
