Elektrokardiyografik olarak sol ventrikül hipertrofisi gösteren hastalarda egzersiz sonu sistoluk kan basıncı cevabının tanı değeri
Sol ventrikül hipertrofisi gösteren hipertansif hastalar, koroner arterleri normal olsa dahi, sıklıkla positif efor testi sonucu verirler. Bu nedenle egzersize bağlı ST segment depresyonları, her zaman koroner sklerozunun bir göstergesi olamazlar. Bu çalışmada koroner yetmezliğinin araştırılmasında postegzersiz sistolik kan basıncı değişikliklerinin değeri araştırılmıştır. Elektrokardiogramlarında sol ventrikül hipertrofisi olan ve egzersiz testinde pozitif ST segment depresyonu gösteren 29 hipertansif hasta bu çalışmaya dahil edildiler. Tüm hastalarda koroner anjiografik tetkik yapıldı. 17 vakanın angiogramları normal (grup I), 12 tanesinde ise önemli (%>70) darlık saptandı (grup II). Sistolik kan basıncı oranı (Eforu taldbeden dinlenme süresinde 3. dakikadaki sistolik kan basıncının, maksimum efor esnasındaki kan basıncına oranı) her iki grupta değerlendirildi. Bu oran ikinci gruptakilerde (oran >0.85), birinci gruptakilerden (oran < 0.80) anlamlı derecede yüksek idi. Testin duyarlılığı, özgünlüğü, doğruluğu ve öngörebilme değerleri sırası ile %76,4 ; % 83,3 ; % 85,1 ve % 86,6 bulunmuştur. Bu bulgulara dayanarak sol ventrikül hipertrofisi olan hipertansif hastalarda koroner sklerozu tanısı konmasında, egzersiz testinde ST depresyonu olanlarda anormal sistolik kan basıncı oranının beraber olması testin doğruluk değerini arttıracağı kabul edilmiştir.
Diagnostic usefulness of postexercise systolic blood pressure response for detection of coronary disease in hypertensive patients with electrocardiographic left ventricular hypertrophy
Hypertensive patients with left ventricular hypertrophy often have a positive result on exercise testing despite a normal coronary arteriogram. As a result, exercise induced ST segment depression is not always an accurate indicator of the pre-sence of coronary sclerosis. In this study, the usefulness of the postexercise systolic blood pressure response for detection of coronary sclerosis was evaluated. 29 patients with both electrocardiographic left ventricular hypertrophy and positive ST depression on treadmill exercise testing were included in this study. All patients underwent coronary artériographie examination. Coronary arteriograms showed normal findings in 17 patients (group I) and significant (70%) narrowing in 12 patients (group II). The systolic blood pressure ratio (systolic blood pressure at 3 minutes of recovery divided systolic blood pressure at peak exercise) was estimated in both groups. This ratio was significantly higher in group II (ratio > 0.85) than in group I (ratio < 0.80). The sensitivity, specificity, accuracy and predictive value of the test were 76.4 %; 83,3 %; 85,1 % and 86,6 % respectively. It was believed that the use of abnormal blood pressure ratio in combination with ST depression in exercise test, improves the accuracy of the test for detecting coronary sclerosis in hypertensive patients with electrocardiographic left ventricular hypertrophy.
