Postmenopozal hormon replasmanında transdermal östradiol ve konjuge östrojen kullanımının lipid profiline etkisi araştırılmıştır. Cerrahi menopoza giren 55 olguda başlangıç total kolesterol (TK), trigliserid (TG), HDL-Kolesterol (HDL-K), LDL-Kolesterol (LDL-K) ölçümleri yapılmıştır. Olgular rastgele ikiye ayrılarak l. gruba (n=32) transdermal östradiol (0.05 mg/gün kesintisiz), ll.gruba (n=23) oral konjuge östrojen (0.625 mg/gün kesintisiz) verilmiş, aynı ölçümler bir yıl sonra tekrarlanmıştır. Transdermal östradiol grubunda ortalama serum östradiol düzeyinde 1.78 kat artış sağlanmış, HDL-K seviyesinde istatistiksel olarak anlamlı bir artış (p=0.008), anlamlı olmasada TG seviyesinde %6.1 oranında azalma, TK, LDL-K değerlerinde değişim gözlenmemiştir. Oral konjuge östrojen grubunda; ortalama serum östradiol düzeyinde 5.0 kat artış saptanmış, HDL-K seviyesinde %7.8'lik, TG seviyesinde %9.5'luk artış istatistiksel olarak anlamlı bulunmamış, LDL-K ve TK değerlerinde değişim gözlenmemiştir. Transdermal kullanımda HDL-K düzeylerinde artış yanında TG seviyelerinde hafifde olsa düşme, oral kullanımda istatistiksel olarak anlamlı olmasada HDL-K ve TG'de artış, postmenopozal östrojen replasman tedavisindeki değişik uygulama şekillerinin lipid profilinde sağladıkları benzer olumlu etkiler olarak değerlendirilmiştir.

The purpose of this study was to investigate the effects of oral conjugated estrogen and transdermal estradiol on lipid profiles in surgically menopausal patients. Initial total cholesterol (TC), triglycéride (TG), HDL-cholesterol (HDL-C), LDL-cholesterol (LDL-C) were measured in fifty-five surgically menopausal patients. The patients were divided into two groups randomly. Transdermal estradiol was administered to the first group [(n=32), 0.05 mgr/day continuously] and oral conjugated estrogen was administered to the second group[(n=23), 0.625 mgr/day continuously]. All of the biochemical tests were repeated a year after the commencement of therapy. Serum estradiol levels were increased 1.8 times in the group using transdermal estradiol. Although there were no significant changes in TC and LDL-C, a statistically significant increase in HDL-C measurements (p=0.008), and not significant decrease of 6 % in TG levels were detected. In the oral conjugated estrogen group, even though the serum estrodiol levels were elevated 5 % times, 8 % decrease in HDL-C levels and 9.6 % increase in TG (p>0.05) were detected. There was no difference in LDL-C and TC levels. As a result, we have evaluated increase in HDL-C and decrease in TG in transdermal estrogen therapy and increase in HDL-C and TG in oral estrogen therapy as beneficial effects of postmenopausal estrogen replacement therapy on the lipid profile regardless of the different regimens selected.
