Status epileptikusa sekonder geçici nitelikte yaygın beyaz cevher, kortikal gri cevher ve derin gri cevher tutulumu: Ender bir vaka
Akut menenjit tablosu ile başvuran 3 aylık erkek çocuk çok sayıda jeneralize tonik-klonik nöbet ve 40 dakika süren bir epileptik nöbet geçirdi. Erken dönem kraniyal BT ve MR incelemesinde yaygın subkortikal beyaz cevher ödemi, giral şişkinlik, her iki talamus ve sağ kaudat nükleusta lezyonlar mevcut olup, bu lezyonlar takip BT ve MRG incelemelerinde kaybolmuştu. Bu vakada lezyonların dağılımı az rastlanan bir örnek olup, özellikle kaudat nükleus tutulumu çok enderdir. Tanımlanan lezyonlar ödemle uyumludur. Lezyonların geçici oluşu ve yerleşim yerleri iskemik lezyonları dışlamıştır.
Reversible wide-spread signal abnormalities in the white matter, cortical gray matter and deep gray matter secondary to status epilepticus: A rare case
We report a case of 3 months-old male who presented with acute meningitis and had experienced several generalized tonic-myoclonic seizures and a single attack of status epilepticus lasting 40 minutes. Early cranial CT and MRI findings revealed wide-spread subcortical white matter edema, gyral swelling, focal lesions in both thalamus and the right caudate nucleus, which resolved in the follow-up examinations. The distribution of the lesions were unique in this case and the involvement of the caudate nucleus is a very rare one. The lesions were compatible with edema. The reversibility and locations of the signal changes helped exclude an ischemic etiology.
