The Effect of Parity, Previous Vaginal Surgery, and Use of Hemostatic Agent on Perioperative Results of Transobturator Tape Operations

ABSTRACT Objective: We aimed to evaluate the parity,history of vaginal surgery, and use of absorbable hemostatic sponges during surgery for their impact on operation outcomes in patients undergoing transobturator tape (TOT) procedure for stress urinary incontinence (SUI). Methods: We retrospectively reviewed the medical records of 53 women,who underwent TOT procedure for SUI in the Department of Obstetrics and Gynecology in Bucak State Hospital,Burdur, between January 2009 and November 2012. All of the medical data including age, parity, complications, the difference in haemoglobin (Hb) levels measured before and after surgery (delta Hb) were retrieved from the patient charts and hospital records. Results: There was not a statistically significant relationship between number of parity (p=0,241) or history of vaginal surgery (p=0,276) and complication rates. The use of absorbable hemostatic sponges during surgery significantly increased the complication rates (p=0,043). There was no statistically significant relationship between the number of parity (p=0,712),history of vaginal surgery (p=1,00),the use of absorbable hemostatic sponges during surgery (p=0,694) and the treatment success of TOT procedure. Delta Hb did not correlate with parity (p=0,161) in our study. Both history of vaginal surgery (p=1,00) and the use of absorbable hemostatic sponges during surgery (p=0,327) did not have a statistically significant impact on delta Hb. Conclusion: The use of hemostatic gelatin sponge has no positive effect on TOT outcomes. Prospective, randomized, controlled studies on a larger number of patients are needed to clearly identify the factors that would raise the success rate and reduce the complication rate in TOT procedure.Post-operative; Outcomes; Stress urinary incontinence; Transobturator tape.
