KORONER ARTER BAYPASS CERRAHİSİ SONRASI ACİL ŞARTLARDA REVİZYON: YOĞUN BAKIMDA MI? AMELİYATHANEDE Mİ? Revision After Coronary Artery Bypass Surgery Under Emergent Instances: In Intensive Care Unit or Operating Room?
ÖZETAmaç: Açık kalp cerrahisi sonrasında çeşitli nedenlerle yeniden girişim gerekebilmektedir. Bu çalışmanınamacı klinik tecrübelerimizi paylaşma ve acil şartlarda reoperasyona alınan hastalardataşınma kaynaklı risk faktörlerini, mortalite ve morbidite oranlarını azaltabilmektir.Yöntem: Kalp ve damar cerrahi kliniğimizde Kasım 2014 ile Temmuz 2016 tarihleri arasında çeşitlinedenlerle birisi ameliyathanede olmak üzere diğerleri yoğunbakım ünitesinde revizyona alınantoplam 11 hasta retrospektif olarak değerlendirilmiştir.Bulgular: Opere edilen hastaların dokuzu erkek ikisi kadın hastaydı. Hastalardan birisi pnömotorakstanısı, iki hasta heparinin indüklediği trombositopeni gelişmesi sonrasında oluşan kanama,iki hasta revizyon gerektiren kanama, beş hasta ani gelişen hemodinamik bozukluk nedenleriyleyoğun bakımda ve bir hasta da ani gelişen kardiyak arrest sonrası ameliyathanede acil olarakrevize edildi ve bu ameliyathanede opere olan hasta revizyon sonrası postoperatif ikinci gündeeksitus oldu. Yoğun bakımda revize edilen hastalarda sağ kalım oranı % 100 idi. On hasta revizyonkararı verildiğinde entübeydi, bir hasta ise yeniden entübe edildi. Revize edilen hiçbir hastadayara yeri enfeksiyonu, derin sternum enfeksiyonu, sepsis, sternum ayrılması gibi komplikasyonlargelişmedi.Sonuç: Açık kalp cerrahisi geçiren hastaların postoperatif dönemde tam donanımlı yoğun bakımdayeniden opere edilebileceğini ve bu şekilde yapılan bir müdahalenin gerekli ön hazırlıklaryapıldığı takdirde mortalite ve morbiditede artışa neden olmayacağını, aksine transfer nedeniyledoğacak riskleri azaltacağını düşünmekteyiz.Anahtar Sözcükler: Açık kalp cerrahisi; Yoğun bakım; ReoperasyonABSTRACTAim: Sometimes it is needed to reoperate the patient after open heart surgery due to variousreasons. The purpose of this study is to share our clinical experiences and reduce the mortalityand morbidity rates and multiple risk factors arising from the transport of the patients undergoingemergent reoperation after open heart surgery.Material and Methods: Eleven patients undergoing reoperation between November 2014 andJuly 2016, one in operating room and ten in intensive care unit, were evaluated retrospectivelyResults: Nine patients undergoing reoperation were male and two were female. The diagnosisnecessitating reoperation in intensive care unit were as following, pneumothorax for one patient,heparine induced thrombocytopenia, massive bleeding for two each patients and fulminanhemodynamic instability for five patients. One patient was operated in the operating roombecause of cardiac arrest and the patient died on the postoperative second day. Survival ratefor the patients ungergoing reoperation in intensive care unit was 100%. Ten patients had beenentubated already and one was reentubated after the reoperation decision. We did not observeincisional infection, deep sternal infection, sepsis, sternal dehiscence in any of the patients.Conclusion: In case of appropriate conditions, the reoperation of the patients under emergentinstances in intensive care unit with general anesthesia may reduce the mortality and morbidityrates and multiple risk factors arising from the transport of the patient.Keywords: Open heart surgery; Intensive care unit; Reoperation
