HUZURSUZ BACAK SENDROMU TANISI OLANLAR İLACA BAĞLI AKATİZİ GELİŞİMİ AÇISINDAN DAHA MI RİSKLİ?
Huzursuz bacak sendromu (HBS) sıklıkla rahatsız edici ya da hoş olmayan duyumların eşliğinde bacakları hareket ettirmek için bir dürtü ile karakterize kronik nörosensorimotor bir bozukluktur. HBS ve akatizi benzer klinik belirtiler ve ortak patofizyolojik paylaşımlar şeklinde görünüyor olsa da, farklı klinik sendromlar olarak kabul edilmektedir. Akatizi, subjektif olarak huzursuzluk ve iç gerginliği, objektif olarak da bacaklarda yarı maksatlı veya amaçsız hareketler şeklindedır. Metoklopramid (MP), yaygın olarak kullanılan, bir dopamin (D2) reseptör blokeridir. MP diğer dopamin reseptör blokeri ilaçlar gibi ekstrapiramidal sistem belirtilerine (EPS) sebep olabilir. Bizim bu yazımız da postoperatif MP tedavisi sonrası şiddetli akatizi (BARS skoru: 9) gelişen 64 yaşında huzursuz bacak sendromlu bayan hastadan bahsedilmektedir. Vakamızın ilginç kliniğinin olması nedeniyle ayrıca HBS'lı hastalarda, ilaca bağlı akatizi (İBA) görülme riskinin daha fazla olduğu ve HBS li hastalarda MP yapılmadan önce bu bilginin de göz önünde bulundurulması gerektiğini literatür eşliğinde tartışmak istedik.
Do the Patients with Restless leg Syndrome Have More Risky to Develop Drug-Induced Akathisia?
Restless legs syndrome (RLS) is a chronic neurosensorimotor disorder, characterized by an urge to move the legs which is often accompanied by uncomfortable or unpleasant sensations. Although RLS and akathisia have similar clinical manifestations and seem to share a common pathophysiology, they are regarded as distinct clinical syndromes. Akathisia, a subjective report of restlessness and inner tension and the objective manifestations of restlessness in the form of semi purposeful or purposeless movements of the limbs. Metoclopromide (MP) is commonly used dopamine (D2) receptor blocker. It can lead extrapyramidal signs (EPS) as other dopamine receptor blockers. In this article, we presented a 64-year women with restless leg syndrome (RLS) with severe akathisia (BARS score: 9) developed due to MP treatment given to prevent postoperative vomitting. Here, we aimed to report this clinically interesting case to emphasize drug-induced akathisia (DIA), the increased risk of DIA among patients with RLS and the importance to consider development of such adverse effect in use of MP.
