Nozokomiyal pseudomonas aerugınosa izolatlarında doripenemin diğer karbapenemlerle in-vitro karşılaştırmalı etkinliği
Ülkemizde yeni kullanıma giren doripenemin in-vitro antipsödomonal etkinliği imipenem ve meropenem ile karşılaştırmalı olarak belirlenmiştir. Ocak-Aralık 2008 tarihleri arasında çeşitli klinik örneklerden ardışık olarak izole edilen ve nozokomiyal infeksiyon etkeni olduğu belirlenen 90 Pseudomonas aeruginosa izolatı çalışmaya alınmıştır. İzolatların 76’sı (% 84) yoğun bakım ünitesinden elde edilmiştir. Doripenem, imipenem ve meropenemin duyarlılıkları E-test yöntemi ile belirlenmiştir. Duyarlılık sınırlarının belirlenmesinde EUCAST’ın belirlediği minimal inhibitör konsantrasyon (MİK) değerleri esas alınmıştır. P.aeruginosa izolatlarının test edilen diğer antibiyotiklere duyarlılıkları ise CLSI kriterlerine göre disk difüzyon yöntemi ile saptanmıştır. Doripenemin MİK50 düzeyleri imipenemden 6 kat, meropenemden ise 3 kat; MİK90 düzeyleri ise her iki antibiyotikten de 4 kat daha düşük saptanmıştır. Duyarlılık oranları ise doripenem, imipenem ve meropenem için sırasıyla % 64, % 61 ve % 58 olarak bulunmuştur. Doripenemin farmakokinetiği ile ilgili olarak uzun süreli infüzyon sonrası 8 µg/mL düzeye ulaştığı göz önüne alındığında izolatların % 91’inin bu düzeyde in-vitro olarak inhibe olduğu gözlenmiştir. Her ne kadar doripenem direnci de yüksek ise de imipenem ve meropeneme göre daha düşük MİK düzeylerine sahip olması ve uzun süreli infüzyon ile daha yüksek serum konsantrasyonları elde edilmesi P.aeruginosa ile gelişen infeksiyonların tedavisinde bir seçenek olabileceğini göstermektedir.
Comparative in vitro activity of doripenem with other carbapenems against nosocomial pseudomonas aeruginosa isolates
The in vitro antipseudomonal activity of doripenem, which is recently introduced commercially in Turkey, was compared with imipenem and meropenem. A total of 90 Pseudomonas aeruginosa strains isolated consequently from nosocomial infections between January and December 2008 were tested. 84 % of the isolates were obtained from intensive care unit patients. Antimicrobial susceptibilities for doripenem, imipenem and meropenem was determined by E-test method. In the inter-pretation of the MIC results, EUCAST criteria were used. The antimicrobial susceptibility testing for other antibiotics were performed by disk diffusion method according to CLSI criteria. MIC50 levels for doripenem were found to be 6-fold lower than that of imipenem, and 3-fold lower than that of meropenem, and MIC90 levels of doripenem were 4-fold lower than those of both antibiotics. Susceptibility rates for doripenem, imipenem and meropenem were 64 %, 61 % and 58 %, respectively. 91 % of the isolates were inhibited at 8 µg/mL concen-tration of doripenem, which can be reached by 4-hr extended infusion. Although the resistance rates to doripenem, as well as other carbapenems, are high in our hospital, doripenem can be still considered as an option in the treatment of nosocomial P.aeruginosa infections because of its lower MIC levels and higher serum concentrations than those of imipenem and meropenem, and its pharmacodynamic properties.
