Genişlemiş spektrumlu beta-laktamaz saptanmasında ikinci, üçüncü ve dördüncü kuşak sefalosporinlerin çift disk sinerji testinde kullanılması ve sefoksitin duyarlılığı
Fenotipik doğrulama testi ile genişlemiş spektrumlu beta-laktamaz (GSBL) üretimi saptanan 170 Escherichia coli ve 40 Klebsiella spp. Suşunda ikinci, üçüncü ve dördüncü kuşak sefalosporinlerin çift disk sinerji (ÇDS) testi ile klavulanat sinerjisi gösterme oranları araştırılmış, aynı zamanda bakterilerin sefoksitin duyarlılığına bakılmıştır. Klavulanat sinerjisi sekiz antibiyotik diski (sefuroksim, sefiksim, sefotaksim, seftazidim, seftizoksim, seftriakson, sefepim ve aztreonam) amoksisilin-klavulanik asit diskinden 25-20 mm uzağa yerleştirilerek belirlenmiştir. Sefepim ve seftazidimin diskler arası aralık farkı gözetmeksizin, tüm suşlarda sinerji gösterdikleri bulunmuştur. Diğer antibiyotiklerin 25 mm aralıkta klavulanat sinerjisi saptama oranları % 12-98 arasında değişmiştir. Aralığın 20 mm’ye düşürülmesi ile klavulanat sinerjisinin görülme oranı yükselmiş, aztreonam, seftriakson, sefotaksim ve seftizoksim ile sinerji görülme oranı % 100 bulunmuş, sefiksim ve sefuroksimde % 88 ve % 20 oranları saptanmıştır. Sefoksitine Klebsiella spp. Suşlarının tamamı duyarlı, E.coli suşlarının ise 162’si duyarlı, 8’i (% 5) dirençli bulunmuştur. Sonuç olarak ÇDS testinde en duyarlı antibiyotiklerin sefepim ve seftazidim olduğu bulunmuştur. Seftriakson, sefotaksim ve seftizoksimin ÇDS testinde birbirlerini tam uyumlu bir şekilde temsil ettiği görülmüştür. GSBL üreten bakterilerin beraberinde AmpC beta-laktamaza sahip olması ve/veya porin geçirgenliğindeki mutasyonlar nedeniyle, sefoksitine her zaman duyarlı olarak saptanamayacağı bilinmelidir.
The use of second, third and fourth generation cephalosporins in double disk synergy test for detection of extended-spectrum beta-lactamases and the susceptibility of strains to cefoxitin
Clavulanate synergy rates of second, third and fourth generation cephalosporins with double disk synergy (DDS) test were investigated in 170 Escherichia coli and 40 Klebsiella spp. strains producing extended-spectrum beta-lactamases (ESBL) detected by phenotyphic confirmation. Also cefoxitin susceptibility of these bacteria were screened. Clavulanate synergy was determined by placing eight antibiotic disks (cefuroxime, cefixime, cefotaxime, ceftizoxime, ceftriaxone, cefepime and aztreonam) at a distance of 25-20 mm from amoxicillin-clavulanate disk. Cefepime and ceftazidime were found to show synergy in all isolates independent from the distance between cephalosporins and clavulanat disk. Clavulanate synergy rates of other antibiotics with a distance of 25 mm were ranging from 12 % to 98 %. Clavulanate synergy rates were increased by reducing the distance to 20 mm. Synergy rates of aztreonam, ceftriaxone, cefotaxime and ceftizoxime were found to be 100 %, cefixime 88 % and cefuroxime 20 % in 20 mm distances. All Klebsiella spp. Strains were found sensitive to cefoxitin. Cefoxitin resistance were found in 8 (5 %) E.coli strains. In conclusion, cefepime and ceftazidime were found as the most sensitive antibiotics in DDS test. It was found that ceftriaxone, cefotaxime and ceftizoxime represented each other with complete accordance in DDS tests. It should be considered that ESBL producing bacteria could concurrently produce AmpC beta-lactamases and/or may have mutations in porin permeability so that, they may not be found susceptible to cefoxitine.
