Mycobacterium bovis’in etken olduğu tüberküloz olguları
Başlıca infekte sütlerle bulaşan Mycobacterium bovis’in sebep olduğu tüberküloz; son yıllarda çok önceden alınan etkenlerin endojen reaktivasyonuyla, HIV-M.bovis ko-infeksiyonu ya da immünsüprese hastalarda fırsatçı infeksiyon olarak ortaya çıkmaktadır. Bu çalışmada laboratuvarımızda 2008 yılı içinde farklı kliniklerden izole edilen M.bovis izolatları irdelenmiştir. Klinik özellikleri, klasik kültürde uzun üreme süresi ve identiŞkasyonda Mycobacterium tuberculosis’den farklılık gösteren üç olguya ait izolatlar spoligotiplendirme yöntemiyle M.bovis olarak belirlenmiştir. Tüberküloz etkenlerinin tür düzeyinde ayrımını yapan laboratuvar yöntemlerinin kullanımının yaygınlaşması gerçek M.bovis olgu sıklığının ortaya çıkmasına ışık tutacaktır. Multidisipliner klinik yaklaşım gerektiren ve akciğer dışı yerleşen tüberküloz olguları için mikobakteriyoloji laboratuvarının daha etkin kullanılması ve klinik-laboratuvar işbirliğinin erken tanı ve tedavi avantajı sağlayacağı, morbidite ve mortaliteleri azaltacağı düşünülmelidir.
Tuberculosis cases caused by mycobacterium bovis
Tuberculosis caused by Mycobacterium bovis transmitted mainly by infected milk arises in last years by endogen reactivation of agents taken long before, by co-infection of HIV and M.bovis or opportunistic infection in immunocompromised patients. In this study, M.bovis strains isolated in our laboratory from different clinics in 2008 were considered. Three isolates from three patients with unusual clinical pictures, grew in loger period in classical culture method and showed different characteristics than Mycobacterium tuberculosis were identified as M.bovis by spoligotyping. The widespread uses of laboratory assays to differentiate tuberculosis agents in species level will provide the real case frequency of M.bovis. For extrapulmonary tuberculosis cases, which requires multidisciplinary clinical approaches, the effective use of mycobacteriology laboratories and cooperation between laboratory and clinics provides advantages in early diagnosis and treatment, and decreases mortality and morbidity.
